So, the game is based on the 1999 movie "Fight Club", and the character in question - Robert Paulson aka Bob - is a former bodybuilder who ended up developing testicular cancer. As a side effect of this, his hormones go out of wack and he develops what the movie refers to as "bitch tits" - hence the feminine body, and why it is portrayed in the game clip.
