Hi David, when risks become an issue, do we remove the said risk from risk register and add it to the issue log instead?

You add it to the issue register, however you don't have to remove it from the Risk register. You want to manage the issue immediately, but still keep track of it as a Risk for the future, including the different possible risk responses 🙂
