Unless it overheats and burns uo or gets physically damaged somehow there's not much that can go wrong with a fused cut off with a knife switch mechanism like that especially since it doesn't have springs or anything that can really break in those. @@jeffreykornspan9053 My comment was only applicable to the line-side service conductor taps, which are legal, if installed properly. Regarding the bonding of the UG metallic service riser; you are correct, the required ‘Service’ bonding is missing. Regarding having the grounded conductor cut in ‘flat’ on the ‘B’ leg of the safety switch, that is NEC legal since the ungrounded conductors ‘open’ simultaneously with the grounded conductor.

@@linemanundergroundworld4854 He means that expansion and contraction of the wire can cause it to wear through the insulation. In my 45 year career as a sparky, seen it happen..but only a few times. Many other times it was finding a split bolt, nut, setscrew or lug in the pipe when pulling the wire out on demo. Should it have been fished out? Yeah, but my experience has shown that it will take years for it to cause any damage. with those conductors in a 4" (I think that's what you said) pipe it's very unlikely to be a problem.
