I think the fact that 4-5-1 attack didnt even turn out to be 89 rated goes to show that the avg rating is calculated over def-mid-attack combinations. Maybe you can keep some of these formations in mind due to this as well. 4-3-1-2 is a sick formation for a high rated team it seems..
