What are your thoughts on Bipella (impella RP and impella RP) when VA ecmo is not available. Also ecpella (impella CP with VV ecmo) Have you seen this?

Have not done BiPella, hence cannot commend on this. Regarding VA-ECMO it is increasingly being realized that afterload reduction helps prevent LV thrombus and facilitates LV recovery. See recent Circulation paper: www.ahajournals.org/doi/10.1161/CIRCULATIONAHA.120.048792 This can be achieved with Impella or IABP.

Why Impella in cardiogenic shock?
