On the issue of how long have sanyasis been called Maharaj; I have heard HD Goswami say that it is not very old. In fact he himself doesn’t like to be called Maharaj and prefers Acaryadev. The term Prabhu, on the other hand, is a very old way to address a Vaishnava. It’s used in this way in Srimad Bhagavatam.
