Wow, this was done on my tooth #3 YESTERDAY for a 3/4 palladium crown. Wearing a temporary as we speak. No idea the prep was this sophisticated. I feel fortunate to have found her-a practitioner with Ph.D in Prosthodontics from a UK university-cause I live in the US and she practices in Southeast Asia. Had a trustworthy referral. I love how smooth and confident you do each prep doctor. This video saved me just like the canine 3/4 preparation ! :) I used to put the grooves first, but It always affected my flare and therefore I used to end up on reducing the labial side. Your method is truly wise and helpful ! Thank you so much!

I did this during my undergraduate years in 2017 and there was a look of interest on my prosthodontic tutor's face when I told her that I was going to do a 3/4 crown preparation. I still do them in practice. It's just a good way to delay the full gold crown on that particular tooth. Great preparation video! I learned a lot especially with the buccal cusp chamfer/offset.

Can you please clarify whether we need to cut 1mm of the palatal cusp and then an additional 0.5mm as a palatal cusp bevel? OR 1.5mm of the palatal cusp occlusal ly then the bevel? However, this will take it to 2mm total. Thank you.

1.5 off the lingual cusp and 1.5 mm bevel - this will not change the B plane reduction but will shorten the cusp to to 1.5 mm

Dr can u tell me please the measurment of the prep please?? Ans is the measurment different if the type of crown change??

1.5 mm occlusal clearance on the functional cusp and 1.0 mm on the non-functional cusp. The offset measures about 1.5 mm wide and have a 0.5 mm deep "v" shaped notch located within the offset. The axial grooves are made with the 169L and are 0.5 mm deep and 0.5 mm above the finish line. The proximal clearance is 0.5-1.0 mm. The buccal cusp (the non-functional cusp on the upper 3/4) is 0.3 mm. Maxillary 3/4 crowns will all follow these measurements. For Mandibular 3/4 crowns, the numbers are similar, however the mandibular crown uses a facial shoulder and bevel instead of an offset.

I wanted to ask about the first step when you start prepping the occluso-facial aspect of the tooth; we are reducing the occluso-facial by 1mm. are we touching the cusp? or are we just reducing the occlusal plane without touching the facial cusp (contrary to a PFM where we are reducing the cusps while reducing the occlusal plane) ?.

You must reduce the cusp to allow for gold thickness, but the amount you remove is minimal - about 0.5 mm, and then the offset provides for additional support of this thin gold area. Finally, the contra bevel is placed mainly to allow for a smooth finish line.

A veneer is a covering. Full crowns are also called full veneers. The gold ones here are used when enough tooth remains and yet it requires structural support. Ceramic veneers are coverings as well, for esthetic reasons.

The finishing bevel may be less than 0.5 mm, and the offset is initially 1.5 mm and becomes less wide after placing the final finishing bevel.
