With difficult IV (as demanded from other specialties if they fail multiple times in a patient) i most often resort to Ultrasound. I frequently find a proper vein a little bit deeper. Kept in mind to use an adequately long catheter and advance the tip of the needle under direct sight for several millimeters to make sure als the catheter is in place. (Dr Hadzic surely mentioned those here.) If you know how to do a central line then this is very similar. I even think that the other way around (e.g. for internists that only rarely place central lines) this would be a very good practice for ultrasound guided puncture.

@sendiulo - 100% agree with you. However, the ultrasound skill and/or equipment is not widely available. While in our service, nearly everyone is skilled to perform ultrasound-guided deeper IVs, in most instances - we accomplish an IV access much quicker with the techniques I am teaching in the channel. Keep in mind that properly performed US-guided IV access requires sterile technique, ultrasound probe cover, etc - so it is an involving procedure. Greetings and thank you for watching!

​@@nysoravideo The creep up technique, I used to advance the catheter when I see blood flush-back and most of the time the catheter won't thread, another tip I don't know if it was from you or from someone else; after just you enter skin find the tip of the need, if it is not straight on the middle of the vein take it out and reinsert it, "with local anesthetic already in the skin or under GA".

it sort of contradicts his other videos, in which he says not to leave the tourniquet on for too long. I roughly expect the tourniquet to be on for about 90 seconds, not much less, not much longer... I'd add know your anatomy, many nurses dont know the names of veins these days, let alone the vicinity in which to find them. I agree not to rush I rather have a good look prior and choose the right vein and ivc for the task. I'm not familiar with leaving the IVC in but why not if it avoids bruising. I remember the first time looking at an IVDU veins with an ultrasound, i couldnt believe the crud lining them !! if you get a chance take a look

Greetings! Neah. I do not believe that I ever said NOT to leave the tourniquet on for too long. Actually, this is the MOST common mistake - taught in schools and books, as if the tourniquet will cut off blood supply and damage the extremity - this will never happen unless you leave tourniquet for hours. However, if I did say something like that, please send the reference - video and timing and will fix. And thanks for watching! BTW WHich tip was the most useful?

I think biggest mistake is that nurses and physicians are hurrying to canulate the first blue line as soon as they see it. What you need to do is to search all possible vein canulation places and choose the simplest and usable one! Even if it’s difficult iv access and you discovered the possible place for canulation , mostly there is always better place that you haven’t checked!
