The first air 6560 crash wasn't caused by icing, but by pilot error because of 4 things: Captain accidentally disabled the autopilot by applying a bit of force on the control yolk, ignoring the FO telling him that they're off course, only recalibrated their compass once, and poor GPWS that activated late

Because of the extra expenses of De-Icing some pilots will take a chance, and lose. There are also two types of de-icing, one is expensive the other not so much. But, finally, after so many de-icing accidents the authorities finally wised up and move the de-icing machines from the gate closer to the runway. De-icing is only good for about 25 minutes then you have to de-ice again. Too expensive for some pilots so they will roll the dice.
