I think he might be. You can love more then one person. You can love somebody and have your life in a state of a barely controlled mess, live on borowed time, money, on stolen affection - which is probably the most unethical thing to do, worse then letting your trouble affect other people in most of other ways. For better or for worse, I do understand Adam. I'm glad he agreed to come clean. Mary does deserve better. I can't say about honesty, because she knew then he started seeing somebody nearby, but she doesn't seem to like it (as a lot people would not), and she wants different relationship, and she does deserve what she wants. I'm glad she demanded answers and in awe of how both assertive and kind she was. I do wish both of them well.

It's a real thing, but not considered a mental illness by scientists and psychiatrists. People who have gender dysphoria feel very strongly that their gender does not match their biology, but this is not a mental illness. The diagnosis is simply a description of the distress this mismatch causes the person who is experiencing it.
