if there are no cards in the graveyards then the scavengers would not get a counter. Per Gatherer, " If the target of the first ability is an illegal target at the time the ability tries to resolve (likely because it left the graveyard in response), the ability won't resolve and none of its effects will happen. You won't put a +1/+1 counter on Urborg Scavengers." So if there are no legal targets then the ability cant resolve. The Witherbloom Command that kills the RIP game three, the milled cards get exiled as the spell resolves top to bottom. Those cards would go to the exile, then RIP is destroyed and goes to exile leaving only command in the graveyard. You cast Witherbloom Command with first and second Modes. It resolves; The Targeted Player Mills (move to Exile), then you return a Land Card from your Graveyard to your Hand. The Targeted RIP is Destroyed (and Exiled). Finally, the Command Spell is put into its Owner's Graveyard. Card(s) Zone Milled Cards Exile Rest in Peace Exile Witherbloom Command Graveyard Incase anyone is confused as to why the RIP goes to exile instead of the graveyard. Replacement effects have to apply before something happens. Rest in Peace going to the graveyard is the move that is going to happen and Rest in Peace's own replacement effect is still on because it is still on the field so applies to that move altering its destination to exile. 614.4.
