Loved this video. I am a new grad PA-C working in family medicine. I just got done talking to friends from school about a pt who had not seen a provider in ~10 years, had a laundry list of problems, and how I felt bad that I could not address all of them. I will implement your approach!! Also loved your plan of care steps. Thanks!

I am an FNP student in my second clinical rotation. I really appreciate your advice. You're a Godsend. I have moments of hope and moments that I'm discouraged and doubt myself. My background is labor and delivery for the past 22 years. I'm just eager to be the best FNP for my patients. Thank you for being a beacon of light.

I especially liked your real world tips. I'm going to try your approach! Trying to address multiple problems can be very overwhelming for the patient as well as the provider, which does a disservice to both. Thanks!

I really appreciate that insight of it's not realistic to address all the problems when patients come in not having been in in several years. Hearing that and your perspective takes a little bit of the pressure off and helped me understand that while you want to address all their needs, reasonably you may only be able to thoroughly tackle a few issues in one appointment. Thank you, your videos are so helpful. I found them when I was looking for help with history intake assessments. I am currently in Advanced Health Assessment (FNP) and I am struggling with my simulation assessments. I understand the OLDCARTS logic and feel comfortable with that, but the initial questions and follow up question I struggle with as well completing unnecessary assessments. Your videos are very insightful, thank you!

Liz, I'm getting ready to start at a Family Practice soon. This video was great! Thanks so much! I find myself getting sucked in, so I have to learn how to not only take care of my patients but myself as well...by not overwhelming them or myself. Thankfully, I'm joining a practice with a very well known and reputable doctor that loves to teach! He also has two other NPs one is fairly new as well. I'll keep you posted, and will definitely be listening and hitting the like button on all your videos. Once I start getting some income rolling in, I'd love to take the Lab course and Kim's diabetes course. Thanks so much for all you do! (Lynn)

How do you chart against the extra problem if you don’t address it?

This is tricky - if it's something that has a plan that's substantial and needs to be followed up on, it should be charted. But some things are conversations and you have to use your judgment there, because not all things like "quick questions" need addressing each visit. I definitely struggled with that at the beginning but I think over time you develop that clinical judgment to discern what needs comprehensive addressing versus is sort of just a conversation
