Well, starting out with "fatigue and pain" was disheartening, but I did appreciate the more clear distinctions of the physical exam that Dr. Bateman presented.

Though I highly appreciate doctors who are willing top spend their time and effort on this 'vague' but nasty disease, I'm worrying a bit about these new criteria; I really hope dr. Bateman will consult patients organizations. Diagnostic criteria are not just to be accepted by doctors and researchers, but also by patients. In my opinion 'core criteria' are not very helpful, this disease is complex, and so will be the criteria. Accepting 'simple' diagnostic criteria just because a proper diagnosis is too time consuming is a defeat. Considering this, I always wonder why there is so little use of (digital) questionnaires and online diary keeping etc. in clinical settings; it seems like the medical profession is frozen in time (for instance: the only place I still get to see a fax machine (for the younger audience: this is a kind of remote printer/scanner) is in a doctors' office or a hospital). Why not define objective criteria, ånd subjective ones, by constructing a decent questionnaire which can be filled out online? Preferably based on input from real patients? The ones which it is all about? The data collected this way also could become a great resource for researchers. It would offer great opportunities for exploratory research. Until now I encountered only one clinic in The Netherlands which has adopted 21st century tools; the Helder Clinic. They save precious consulting time by sending a questionnaire to their patients beforehand, and they keep an online medical record which can be accessed at all time by patients and their doctors. Though they are not specialized in CFS, they are a good example of how things should be. I sincerely hope dr. Bateman won't forget to include the 'subjective' complaints of her patients in defining new criteria. The emphasis on ''signs'' opposed to ''symptoms'' has become epidemic in the medical field by now, it's time to go in reverse, to make progression.
