Putting "xiao" at the beginning of a name is kind of like the "-chan" suffix in Japan. It indicates something is small and (usually) cute. Concubine Gyokyou's daughter is named Princess Lingli, but Gyokyou frequently calls her Xiaoling, a diminutive of her name. In the same way, Xiaomao is a diminutive of Maomao. One spelling of mao in Chinese means cat, so Xiaomao means kitten. It's funny when Gaoshun calls her that because he went from calling her Maoamao-sama to calling her Xiaomao in less than 10 seconds lol

Iirc: The anime glossed over it a bit, but in the source material they clarified that Gyokuyo is actually in love with the emperor. But because of his position he has to have multiple concubines. So she's jealous of Fuyo because she can keep her lover to herself, while Gyokuyo can never do the same
