Dr. Lee, as an optometrist, while I don't do many sharp blade procedures, one thing I would suggest is to never point a sharp instrument towards the cornea if you have an alternative. For this patient, with the milium near the inner canthus of the patient's left eye, even though it wouldn't be your normal way, I would suggest coming in from above, with the blade of the scalpel be pointed down...away from the cornea, towards the nose. The same thing with most of the others, keep the sharp edge, or the pointy edge, pointed away from the cornea. We always gotta worry about the patient doing something unexpected like pulling away or turning their head suddenly.
@carrieannewatkins29582 жыл бұрын
I agree. I was watching and thought the patient was going to move forwards a few times. Which is totally understandable. Dr Lee was really sweet etc, but I hated the scalpel pointed that way.

Seeing milia removed is so satisfying! A couple months or so ago, I managed to extract what looked like a small milium in the very corner of my eye, it was more over the bone where the eye-socket begins, before it rises over the nose. It was a scary idea, but with a little stick from a sterilized needle and my home extractor it actually came out painlessly. But it was more liquid than the milia we see in Dr, Lee's videos. More like a whitehead, maybe. So glad it's gone!

The Dr. Won't like me saying this, but you can remove these at home with a sharp sterilized needle. I come in from the side and try to get underneath it. I sometimes have to deal with a small milia myself at home. A 33 gauge blood sugar testing lancet works perfectly to ‘stick’ the milia without it managing to elude all efforts to do so. For the one she had on her eyelid, you can actually just slide a (brand new/expertly sanitized) nail clipper over it and *click*. There will be a bit of blood but chill the area with an ice cube and constrict the capillaries with topical benzocaine ("Oragel") before hand to extremely reduce this. As a bonus it will reduce any pain. Then dab some neosporin on it and apply a bandage. I had to do it for my fiancé once, worked like a charm.
