This is severe atelectasis with a large attic retraction pocket. This stems from long term untreated eustachian tube dysfunction which generates a lot of negative middle ear pressure and sucks in the TM to the point where it drapes over the middle ear structures. Great look at a bad ear pathology Nurse Richard!

Atelectasis normally relates to the lung, is it also used to describe ear dysfunction or pathology?

@@pennycandyys That is correct! We usually see atelectasis of the lungs post-operatively or in cases of pain causing decreased breathing effort like rib fractures. Atelectasis also can be used to describe an under ventilated middle ear space and when it is severe it looks like the ear drum has been shrink wrapped over the middle ear structures like the hearing bones.

Hello Richard, I have to say it’s pure luck that he came in your office when he did. That ear looked horrendous & even the one you did work on wasn’t good either. You have done a fantastic job with the help you gave him. I'm a patient with otitis media. I’ve seen a photo of my eardrum indented on the side of the ossicles. Compared to that, the patient in the video is truly a horror show😱😱😱 05:10 And thick earwax, too.

What is the difference between the Cawthorn (I think that's what you said) hook and the St. Bart's hook? They looked almost identical to me except that I think the Bart's hook had a sharper, more 90-degree angle to the end. Is that the only difference? This stuff is fascinating. Thank you!

That left ear almost looks like you could have sold the contents to make birds nest soup. 🤢 And that right ear is basically unspeakable. And you say he just walked in off the street, he hadn’t even made an appointment to get that horror show looked at? The guy’s like a farmer-they never come in unless they’re at death’s door. I wonder what he considers serious enough to come in for then? We had a patient once, I think he was maybe bragging a bit that he’d never been in to see the doctor. I was like….’well yeah…but now you’ve got cancer everywhere. If you had come in though, maybe it could have been treated. 😬 **Men-go to see your doctor at least once a year.** At least so they know what your normal is, so they know when something is abnormal.
