The grand opening of Chicago Med's emergency department is interrupted by victims of an "L" train crash, pushing the staff to their limits.
From Chicago Med Season 1 Episode 1 'Derailed' -. Dr. Will Halstead and the new trauma fellow Dr. Connor Rhodes butt heads when the latter pulls rank when it comes to treating patients. Medical student Sarah Reese's first days on rotation take an emotional toll on her. Halstead and Manning are forced to perform an emergency surgery on a patient who is a surrogate to a couple.

Breaking someone’s ribs while performing CPR means you’re doing it right. Remember, you’re beating their heart for them. They’re already dead, you can’t do anything worse to them.

Exactly this, i was thinking that the entire time. If you ain't hearing at least a bit of bones breaking, then they WILL be more likely to die. Ribs can heal; a heart that stops pumping can't heal, so you need to force it to pump

can understand though especially on a child why as your learning to become a doctor you will be hesitant. think of the years she has spent learning knowing full well how fragile a childs body can be. it was great praise by the other doctor though to really reinforce that she saved the childs life so dont think about it.

Every surrogacy legal document has a clause stating that if the choice comes between choosing the mother or choosing the baby's life, they must choose the mother. Unless that surrogate purposefully asked for that clause to be left out, which I doubt.

This is why surrogacy is so tricky to word correctly. They said they had ‘medical power of attorney’, but I bet if you got a team of high priced lawyers to tear through the contract, they’d find a way around that so that doctors can override the medical power of attorney in cases such as this one. Or even the hospital lawyer might use their brain and say ‘if mum dies, so does the baby… so do the damn surgery, never mind if the parents try to sue.’ And then there’s the part where a lot of the time, they don’t really fully comprehend what they’re signing and how much power they’re giving the bio parents. Or they never dreamed that a situation such as this one would arise, and the parents would literally making that life or death decision, and choosing for them to die unnecessarily. This is why surrogacy isn’t actually legal in many places. Plenty of people do it anyway, but if things go wrong they have no actual claim to the child, even though it’s biologically theirs. Because surrogacy is illegal, therefore the contract itself is void.

@@dmf1301 idk where you live, but it’s very legal and common in the US and the bio parents have full rights to the child. The child belongs zero percent to the surrogate mom and that’s spelled out pretty clearly in the legal documents. All these dramas where the bio parents have to fight to get their child because the baby “belongs to the surrogate” would never actually happen in the US.

@@emilybeaty27 doesn't change the fact that if they let the surrogate's brain swell, she dies, and if she dies, so does their child. if the fetus dies in the course of saving the surrogate they can get over their narcissism and adopt a kid from foster care.

​@@emilybeaty27 There are some states in America where surrogacy is in fact illegal. The legal documents to say the child belongs to the intended parents and not the surrogate have to go through court. If not approved in a certain amount of time, or not approved because they weren't filed correctly or not in a timely fashion, the surrogate is the legal parent. It's rarely the case, but it does happen.

@@ButtonsCasey I guess we should define terms here, because technically I’m talking about gestational carriers, someone who has no biological bond to the baby. Being a gestational carrier is legal everywhere in the US. A traditional “surrogate” (which isn’t what most people are talking about) is where a couple pays a woman to sleep with the husband (or be inseminated by him), have a baby, and give the baby to the couple, despite the baby being genetically related to the random woman. That is “traditional surrogacy” and is hardly ever done anymore in America and yes, is illegal in some states.

Apparently the little girl ends up being fine, the brain dead guy's lungs are a match for another patient and the parents consent to the surgery on Gina. While the baby goes into distress midway through, they are able to stabilize its vitals and the surgery is a success. Gina later regains consciousness.

When the child arrested, my heart dropped. Reese saved a girls life. Yes, she might’ve broken some ribs. But, the more important part is that she’s alive. Reese did an excellent job handling it.

I agree. I can understand why she's upset about having broken a rib, though. Broken ribs are painful, and doctors are there to heal, not harm. But in those circumstances, she did what she had to do. Broken ribs are painful, but they will heal. A brain starved of oxygen for too long will not heal. So that's why she won't get into trouble for having broken the rib - it's an unfortunate sunk cost fallacy in that circumstance. And I'm sure she'll forgive herself in time - I think this sort of things has happened to many doctors (in the real world, I mean), and a bit of guilt is probably common.

I haven't watched it yet but if the ribs broke due to cpr that's normal. If ribs don't break they aren't doing cpr right, you have to press hard to manually beat the heart that is being protected, but doctors also need to take into consideration what will do the least amount of harm.

@@Bunny-bo2sr People with OI (brittle bone disease) and a few other conditions are far more vulnerable to having broken bones than the average person. So you have to be extra careful if doing CPR on people who are vulnerable, but it's very likely you will cause damage whilst you're trying to save them from worse damage. :(

It's very scary performing CPR but definitely 100% more when you do it on a child especially if it's your first time. Adrenaline kicks in and , even if you break some ribs, seeing a heart rate come back makes you feel better. You still feel bad for breaking ribs but that is nothing compared to how you feel if you lose a patient. You know they are going to be in pain from broken ribs but they heal and giving family back their child does really ease your guilt and there is guilt when you break ribs. Performing CPR is absolutely exhausting and that's why medical staff, tap in and tap out so that the patient is getting the best/correct compressions every single time. Again, adrenaline kicks in and though you feel you can continue to do further compressions, it's more important that the patient gets the right and best treatment so tapping out is very important. Just 1 cycle of CPR can feel like a 30 min weight work out in a gym and though you may hurt, it's always worth the pain, whether the outcome is good or sadly bad as you know you gave it your all

I love how Sharon brought that woman back down to earth about the reality of the surrogacy situation. The fact is that any woman who agrees to become a surrogate for any couple is implicitly agreeing to do what is in the best interest of the child but they are also requiring that their health be treated as equally important as the unborn child. For that couple to treat the woman as nothing more than an incubator to be broken if it meant saving the unborn baby at all costs was just cruel and inhuman beyond all comprehension. They were perfectly willing to let that woman die if it meant their child had a higher chance of surviving. I can't imagine how that woman would feel if she found out those two were ready to let her condition worsen just to save their child, because she was basically being put to death by those two.

You must not have watched the entire episode. They struggled with the decision she wasn’t just a surrogate she was their friend too. To be honest they didn’t make any decision that a parent wouldn’t make if they had to choose between their child or someone else, most people would choose their child no matter how difficult. To be honest I find it messed up they tried to shame those parents for putting their child first. Medically they did have medical power of attorney and in a real life situation they wouldn’t dare try to over ride some one’s power of attorney.

Legally speaking the couple can have their rights to power of attorney revoked in this situation. Because it is law in surrogacy contracts that if the woman's life is in danger and there's no possibility to save both her and the fetus, the woman is to be saved. The only exception is if the surrogate herself gives a DNR and specifically asks for the baby to be saved. Even if they tried to sue the hospital they'll lose because the law is on the side of the hospital.

That’s why I’m against surrogacy. Especially because some couples completely ignore the surrogates wishes and medical decisions, and because they’re the biological parents to the child the law sides with them. It’s so sad, surrogates should not have to almost die in order to fulfill someone else’s wants

Considering she gave them medical power of attorney that’s different than typical surrogacy contracts, no the law wouldn’t be on the hospital’s side. Most surrogacy contracts don’t assign power of attorney to the biological parents. People really shouldn’t weigh in on the topic when they don’t know the parties involved. It’s always possible the person already made their opinions felt on what to do if it comes down to me and the baby. All women once pregnant should have some kind of legal document drawn up specifying their wishes that way it’s not a guessing game or a fight.

I can’t believe they would choose a fetus that has no memories over a young woman that probably has a family, friends and memories. It’s selfish. Adoption is always an option. There are so many kids that just want a home. Maybe there’s a reason why they can’t have kids. Clearly it’s not working out.

My first thought was she didn't look far enough along. If the doctors cant save the girls life because they don't allow a life saving surgery they still would loose the child. If I was in their situation I would choose to let the doctors do the surgery. Hopefully the doctors can save both lives. But since that fetus looked to young to live outside the womb then why risk the woman's life by not letting her have a surgery.

Yes, Sara Reese broke a little girl's ribs . But she saved her life. Chest compressions often involves breaking ribs. She's alive. That's what matters.

I've had broken ribs. Not fun but not the worst break either. I'd happily take broken ribs over heart failure. If you do chest compressions on older people you can break ribs too.

Adoption comes with a lot of legal headaches, expenses, can take a very long time, and often the adopted child has trauma that not everyone is equipped to handle. It’s not for everyone.

@@milanwashington4276 Babies can have trauma too - look up Levi’s Korean adoption story on KZbin, the family does a wonderful job outlining the grieving process babies go through. Newborn adoptions are not common anymore; the vast majority of women who have an unplanned pregnancy in the modern era either abort or choose to raise the child themselves. International adoption is a different story but by the time all of the paper and legal work is processed the child is no longer a newborn.

Just one of the many problems with surrogacy. That woman isn't an incubator for your baby. Do what's best for the mom (regardless of biology, that woman is a mom) and baby.

Which is why I think it should be illegal in all forms. Couples can adopt but surrogacy is an outdated practice built from sexually and reproductive exploitation and even slavery for women.

I was taught that if you haven't broken or cracked some ribs; You didn't do compressions or CPR correctly. Reese did great! I would be relieved if she was my daughter's doctor

Oh this was the first episode? 😂 that explains so much. I’ve seen so many of these I was sitting here wondering why Reese (the student?) was acting all worried? I was so confused. Not to mention there’s just so much going on and even some of the other doctors seemed a bit different

@@scarletohio3528 That’s not true at all, for a couple of reasons. 1) Abortion isn’t blankety illegal or legal in the USA anymore. It is state, hospital, and physician dependent (some states, hospitals, and physicians refuse). 2) If the mother or in this case, surrogate, carrying the fetus dies, it’s not considered an abortion legally if the fetus dies. That would be fetal death or stillbirth. 3) These cases are EXCEEDINGLY rare. They would need to consult the ethics committee, an OB/GYN, and have consent from the family to keep the mother on life support to let the fetus gestate. Also, not all hospitals have the capability to do this. In this situation, no physician would get in trouble for “killing the baby”. The mother is dead and the fetus usually dies along with it unless it’s gestated long enough to be delivered by caesarean or they keep her on life support. Your reasoning is incorrect and misses a lot of the nuances of the situation.

For one thing, if the father is present, then saving the baby can help him cope with the loss of his wife/gf. As that baby is a part of her and can help him remember the good times and help him heal. In a lot of cases, women when they become pregnant will adjust their will/power of attorney statements. Saying that if for any reason they become brain dead and there is a chance to save their baby, then they give doctors the authority to do what they need to. I know this might sound silly, but you'd be surprised how many women, once they become pregnant after having trouble conceiving, are determined to have their baby even if it costs them their life. It's usually only a problem if the state orders the hospital to put the woman on life support. There was a controversial case in Texas when after a pregnant woman ended up brain dead after an accident, even when her husband and family told the doctors to take her off life support. The local judge ordered the hospital to keep the woman alive long enough for the child, a girl, was developed enough to be delivered safely. Lawsuits came flying in saying that the woman specifically had it in both her will and her power of attorney via the husband that once declared dead, even brain dead, the doctors have to take her off life support. And they sued the state for violating the woman's right to medical care. The Supreme Court of Texas ultimately decided they didn't care, they were only focusing on saving the baby; not giving a damn about the family or even what happened to the child afterwards. And this was before Roe was overturned.

It is true that CPR can cause its own set of problems. My cousin was working out at a gym and dropped dead from heart failure. People at the gym did CPR until paramedics arrived. He ended up in ICU for a month not because of his heart, but damage to his lungs from the CPR (I forget the name for it). We almost lost him. He was out of work for months. Granted, if they hadn't done CPR he would have died, but it is true that CPR can have other, fatal, repercussions.

9:18 I can’t imagine the heartache that goes with trying to have a kid for ages, and just never having luck, but like…adoption is always an option. Cuz I mean, there’s always been this romanticized ideal that a family has to be biologically connected. If you REALLY want a family and you and your partner aren’t having luck with pregnancy or a surrogate, try pursuing adoption. A kid shouldn’t have to look like you for you to love them.

If I were in a situation of being incapable of having kids I would inmediatly go to adopt a child, because I know a father is not the one who shares the same blood with you, but the person who raises you, educates you, teaches life lessons and values to you and is always there for you if you need him

I get what you're saying, but that's not always in the best interest of the child. Some people who adopt as a 'last resort' never truly bond with the child they take in and can even grow to resent them for not being biologically related to them. It's awful and unfair, but it does happen.

Interesting how the "mother" was saying there were no more eggs, embryos, etc. and how it was their last chance when I can bet dollars to donuts they never even considered adoption. They'd rather let the actual mother, the surrogate, someone with a family and lifelong memories die than give up something that hasn't even been born yet. Sounds selfish to me regardless of their contract.

Legally speaking, since the surrogates life is in danger, the couple can have their rights revoked. It is law that in the event that a surrogate is in a life or death situation and the doctors had to choose her or the fetus, they must choose the surrogate. Unless the contract specifically says the surrogate agreed to have the fetus saved and she has a DNR order. Even if the couple sued they'll lose.

it’s so sad. honestly in my opinion the ivf/surrogacy industry is messed up and selfish. And I totally agree with what you said about adoption. These parents care more about having a mini them and fulfilling their wants instead of actually loving a child no matter if it’s biological or not

@@laurenorourke2442 Agreed. If it were up to me, surrogacy pregnancy would outlawed as it has a lot of ethical and immoral issues behind it. As for IVF, since it's the woman herself trying to get pregnant, not relying on another woman to do it, I'm more willing to allow it. When it comes to adoption, the process is extremely complicated and a lot of times bs technicalities prevent good people from adopting children. So for them, IVF is more simple, easier and less expensive.

Just know that they would continue compressions until they find a weak pulse on the machine, then use the shock pads to shock the patient into a normal heart rhythm

The contracts I’ve seen state “power of attorney “ for medical reasons. But it also states that if the mothers life is at risk it goes out the window. I wouldn’t put it past some shady people to leave that out though.

Do you know how difficult it is to adopt in the USA? It actually is probably more expensive than using a surrogate and some people want their own biological child. Adoptions fall through all the time, sometimes the child is very violent, many times if you go overseas they have serious health conditions. “Just adopt” are statements that come from people who have never even looked into the process and how hard, strenuous, lengthy, and costly it is. Also, many people don’t even meet the requirements to adopt.

He stayed with her and my family for years, long enough to where he calls my cousin 'mom' and my aunt 'grandma'. He was only sent back to his real mom for literally a week before he was sent back to my cousin. Because the mom's boyfriend tried to kill him. After that, his mom lost all chances to get her son back, and my cousin proceeded to adopt him as her legal son. We love him dearly and consider him a part of the family.

My comment wasn’t intended to say that adoptions don’t occur and good outcomes don’t happen but to say that saying “just adopt” is incredibly tactless. A lot of people do not understand how difficult the process is. Also, your cousin was a foster parent first. Most people who want to adopt are not and have to go through a similar vetting process where it’s already done for foster parents and they are familiar with the system. Not all people who want to adopt want to foster. My point is that it’s wonderful that it worked out for your cousin but that doesn’t take away from the reality that plenty of adoptions fall through and the cost is enormous. Foster to adopt is quite different than adopt from scratch including the cost.

​@@Kenya368 specially because you need to have a financial situation to make sure the kid can be provided for all the things he lacks, plus another conditions such as being mentaly and healthy capacitated to adopt, which means being forced to let third people to look inside of your familiar backround, and if there is any disease or condition no matter how insignifican could be they will reject you because of the slighiest posibility of inheriting those conditions

Legally speaking, since Gina's life is in danger, the couple can have their poa revoked. Fertility clinics that deal with surrogacy have clauses written in their contract that in the event the surrogate is in a life or death situation. Unless it is specifically in the contract that the fetus is to be saved and it is made by the surrogate. Then the doctors must save the surrogate regardless of how the bio parents feel about it. Those doctors should of gotten the hospitals lawyers on the phone as soon as it became obvious Gina needed the surgery to live. To explain to the couple that their rights can be revoked if they refuse.

I am only 13 but here is one thing i can tell you. If you are doing cpr on anything, DO NOT worry about breaking ribs. Its better to resuscitate someone and break a few ribs than to not resuscitate the person in time because you are trying to be careful. If you take too long but succeed, there is a high chance your patient can be brain damaged or die. I am no doctor but thats just some advice.

The heart is bellow the sternum, that long bone in the center of your chest. You gota push deep enough to get the heart to beat. Sometimes ribs break especially in old people. Sometimes you gota break a few ribs to to cpr. Which would you prefer grandma dead or grandma having a broken rib or two.

If the couple decide not to allow the surgery, then the hospital will have to use IV fluids and an incubator to keep the surrogate mother alive, brain dead or not. The incubator provides oxygen and the IV fluids, nutrition to continue fetal development, until the baby is developed enough to safely deliver. After that, the mom is taken off life support. As long as they can keep the surrogate breathing and her organs functioning, the baby will continue to grow and develop. Whether or not it’s ethical or humane is an entirely different discussion

You don't ALWAYS break the ribs when performing CPR. Yes it can happen, but it's not a guarantee. And hurting someone when you're trying to help them is terrifying. Especially when it's a child. Lay off, yeah? It's a pretty realistic reaction.
