I think the man doesn't know how value of his life so he just lives day by day, he doesn't care (smoking )but when he had an acute attack he felt scared and started to value his life and when the auto driver ask him what happend he is thinking how grateful he is that attack was acute and the fact he can create a life (his child ) himself
