You make this look so easy. The antecubital fossa vein I have missed when it is easily palpated. The position of the fossa may bend and not allow the cannula to advance. We need IV's fast due to patients have STEMI's and strokes. Still new to IV's .

If you are having difficulty finding a site, try using either three torniquets or a esmark bandage. Even when I see veins and they look small I use the three torniquet technique and I usually can get a larger catheter in the vein.

Thanks for all this video. Do you have tips on how to avoid veins to roll during cannula insertion? I find it easy to avoid with butterfly needles but the same doesn't apply with cannula

Great question.. 1) tension on skin 2) look at where it rolls and reangle needle 3) remove needle from skin and reinsert very slightly toward the direction it rolls 4) accept that they may roll no matter what you do… and dont persist at same location.. reinsert re angle, try completely different site

A tip - put a couple steri strips on the canula before your tegaderm/ opsite. That tegi will not hold in the long term, especially if a patient goes for a shower / physio / mobilising post op. As a surgical nurse I can't tell you how many canulas have been accidently dislodged due to a lacking dressing. I often redress them straight away with steris and iv3000. For even more reinforcement loop one steri around the canula and then stick it down.

Completely agree. The ones inserted by docs tend to not always have been placed with longevity in mind, which is understandable considering surgery, codes, etc. Before I was allowed to do my cannulation competency module, I’d also redress these cannulas and do the steri strip or sterile tape strips (like the ones in IV3000, unfortunately Teggies don’t have them) around the hub with the ends crossed over each other, kind of like 🎗️, with a Teg on top. These days I’m in Day Chemo, so no need to worry as much about making it crazy secure for days. Though many of our patients have terrible veins from the chemo.

Love your videos & techniques! Could you doe videos on how to heal the veins in people who undergo frequent vitamin C & nutritional IV therapies at an average of 2-3 IVs per week regularly for various purposes. Some veins esp the AC which used to be prominent has gone into “hiding” & painful. Also some videos on why during IV somewhere midway the drip stops on its own while it had been on at slow speed.

Great video. When you're initially approaching the vein, and have your small finger resting to the side for stabilisation, is the cannula advanced into the vein by moving your arm forward or by using your fingers or wrist or all three subtly? thanks for the great content

How do you disinfect your tourniquet between patients? Is it a woven material?

With veins that are only palpable and not visible, how do you remember the location/trajectory of the vein? Do you remember landmarks or anything of the sort? I always have to re-palpate and then insert the needle proximally, where the area is still aseptic 😅

I agree it’s hard to remember the exact spot after sanitising the skin. I try to keep my eye on a freckle or skin crack, but another nurse showed me how she uses an alcohol swab as an indicator! You point one corner of the square swab a few ml from the planned insertion point

I have a question: I have noticed that when I put in peripheral IVs, I usually indent the wall of the vein quite a bit before puncturing it. By the time I get primary flashback, I also frequently have secondary flashback in the cannula itself. At that point, I just thread off the cannula and it seems to work just fine. Watching all these videos, I never see that happen to you. I wonder if I'm doing something different or wrong that's causing this issue? 🤔

Hello! Paramedic here, some catheters (cannulas) have a mechanism to where you see flash in the cannula and the flash chamber around the same time. So I wouldn’t worry about that, as for the indent, it’s important to use traction to hold the skin taught before puncturing, this can make the needle pierce far easier especially through stretchy and loose skin. If you’re doing all this already there’s a chance your hospital or agency may be using cheaper IV start equipment that has dull needles! Cheers mate best of luck

And does it help to 'loosen up' the hub of the cannula by twisting it just a bit while on the needle just before insertion? I've seen some do that and wondered what the manufacturer intended. As youre inserting as long as your fingers aren't in the way its quite easy to notice the flashback. Just pushing the cannula up a tiny bit to "loosen it" and then returning back is done by some docs as it can make pushing the cannula forward easier, but remember to ensure the end of the needle is poking out still , if after loosening it you return it back to normal should be g.

When you cannulate a very superficial vein (say one you can see in the forearm but can't feel) or a small hand vein, do you advance the needle a little more after flash or just advance the cath right away? I ask because I normally advance a little more on the superficial or smaller veins after flash but end up blowing them quite often. Any tips?

You have mentioned about vein stability, something spark into my mind as an idea speaking about the 4 Horsemen. I think there's a great possibility they aren't Kings. Because Kings were enthroned in their areas. The real 4 Horsemen and not the assuming fake 4 Horsemen who want to be famous and be feared upon, could always be on the move, riders or something using the active transport of the Positive Feedback Mechanism but could be not pure positive in being and they have real influence other than the Arab Kings. Could be using parallelism to distance themselves just like Lucifer and Abaddon always have covers and doubles. Even Nimrod, Alexander the Great, King Solomon, and Zeus head of the Greek wisdom mentioned in the time of Antiochus always want to operate behind the screen and camouflage.

I've timed your iv insertion to fat patients with almost no visible veins, the longest time was 2 seconds, the mid time was 1 and a half seconds and the fastest was 1 second to a very fat patient. I think in the category of experienced IV inserters you will be very difficult to beat by them, you can beat them with your inborn skills, unless there's another one as fast as you do that.

There's a wonder in gifted person's, it could be on what's happening in their brain waves that make them function well in a controlled relaxed state of being or in a quick wit mindset as if picking ideas or analyzing things fast or with quick reflex as if it's 2nd natured normal to them. Another is the maximization of potentials due to their body organs was designed as such instilled through biological means since child birth which is quite edgy than normally built person, another is the genetic factor of with harnessed great traits with quite excess combinations of recessive traits( from both parents who excels well in their performances in various areas ) wherein when combined in chromosomes and DNA from parents and from grandparents of generations creates a superior human being in their offsprings. Because what a person eat and the environment and the genes are contributors to an extra gifted person, another is the nurturing of an already hightened skills, knowledge or talents depends on what drives a person to be motivated, enthusiastic, goal driven etc., Another is by the help of outside causative forces like energies, or accidental event which instead causing them deterioration creates a building blocks of improvement, to betterment, to superior being that branches as the brain creates new neuronal branches in learnings and as genetic mutations creates branches so is the human improvements creates branches etcetera that's why the best way to control is superb assessment of a person's capacity without outside biases and distractions by being true to oneself by knowing oneself strength and weakness as an analyst can guide us to better understand the hidden qualities of others as a start wherein when we are in that high level of understanding already we can spot the quality of others wherein that person we are observing is not even aware of his or her excellent side we will have just to open the blind side of him or her to maximize his or her being. Example, a shy person could be confused within we have to give him or her direction to find the way to discover ones being then when a person finds his or her way to his or her nature, we can observe how deep the person is, the deeper the more hidden gifts there are within like analyzing the deep things un analyzed by common people or well studied people, or their memories in their brains are superb, or with brain waves that can connect to the secrets of science, or if his or her inner anger if there's any that will motivate that person to strive better upon our redirection to that person for the better. Another example a fast talking person with sensible words means his or her brain neurons are transmitting organized impulses at a fast rate his or her neuronal branches can have plenty of neuronal lights going in in there, an idea plenty person, quick wit, quick organizer as such. Think of varieties and be open minded because if a person will be true to his or herself and can control his or her being, that person can better control others someday as his or her knowledge progresses.
