Gone through something similar last October I got a small tear in my MCL in my left knee and just this week I feel I can get back to my left side and shot a decent score for me 87 with 4 triples 😂

Tom. A few lessons with a pro and you could really limit these errors. Most are swing related. Get some lessons on your swing and remove these hooks and bobbles.

Tom you lay the club off way too much to be consistent. You’ve got a good straight left arm most of the time. Start working your wrists to be stronger while you make your swing more upright. Keep the turn and the semi-bowed wrist through impact. Simple swing thought would be “keep the body turn, hands higher, then drop the club into the slot”
