Acne Vulgaris: What is it, Risk Factors, pathogenesis, Types (Acne Fulminans, Conglobata & Excoriee), Diagnosis, Treatment, and Complications. Acne is a very common, often chronic, skin condition that leads to non-inflammatory and inflammatory skin lesions. There are 4 pathogenic causes of acne, including infection with the bacteria propionibacterium acnes (also known as cutibacterium acnes). There are also several medications that can cause and/or worsen Acne as well, including phenytoin among others. We discuss influencing factors that can worsen acne as well, including elevated androgen levels. We also discuss the different presentations of acne including acne fulminans, acne conglobata, and acne excoriee. Finally, we discuss the severity, diagnosis and treatment of acne.

Per Cunnliffe and Layton - scarring may occur with mild, moderate as well as severe acne. Therefore early aggressive treatment is recommended. If clearing is not achieved within 3 months treatment needs to be stepped up. The only treatment demonstrated to produce long term remission is oral isotretinoin. However many patients treated with a full course of oral isotretinoin will need to use topical retinoids, benzoyl peroxide, or combo topical product to maintain clear skin afterwards.

Oh my god mam can you please respond to this I NEED HELP so my dermatologist prescribed me 20 isotretinoin tablets and some other creams like azleic acid and a moisturizer I used it religiously and my skin got clear and beautiful in just 3 months(that was the prescribed course) so I stopped using it afterwards however I kept using the moisturizer furthermore cos I liked it. After a year now my skin had started redeveloping these pimples and some are even larger than before.... I am really really confused please help me find a permanent solution for this torcher

Many thanks for this excellent resource. I'd like to add that acne fulminans is a rare variant seen in teenage boys, and its treatment is different. This is because it is an immune reaction to P acnes. The lesions become necrotic, with haemorrhagic crusts surrounding the lesions, and systemic features eg malaise, fever, arthritis are also associated with acne fulminans. You can even get bone lesions from osteolysis. The treatment is systemic steroids (not surprising giving it is an immune reaction) following by oral isotretinoin.

my acne sounds just like yours. Doxycycline has been very effective and inexpensive for me (would have to cut out dairy for most effective results) but you need it prescribed. The only issue is that it's usually not a permanent fix. I also use PanOxyl wash for my face and body which is the only wash that's made a difference on my skin. I haven't cured my acne but i want to say i have controlled it for the most part. Good luck brodie you're not alone.

Hi Dieter Schweiger! Thanks for the comment! Looking back at this lesson, I realize that I should have said that statement "No significant evidence on dietary influence" somewhat differently. There is some evidence showing some association between dietary choices and increased acne. These dietary choices seem to include dairy milk and high glycemic foods. There are still some questions as to how much reducing intake of high-glycemic foods directly reduces acne or if it is the reduction in BMI (that accompanies the decrease in these foods) that is more of a direct link to decreased acne. There are some questionnaire-based retrospective studies that showed some mild association between dairy milk consumption and worsening acne; however, other studies did not show an association. So, if dairy consumption is associated, it's probably a high amount of diary consumption that is the associated cause. At this point, I think we are still in early stages of research for finding associations between diet and worsening acne - if diet is associated, it's probably related to changes in BMI. Hope all of that helps!

I agree with you, Dieter, I know when I changed my diet and stopped eating so much sugar and milk products I didn't have a problem with breakouts anymore. I'm surprised there isn't more evidence for this besides personal or anecdotal examples. I'm convinced that changes in diet can help some people's acne.

great video. I did have 3 considered autoimmune diseases: acne, asthma and severe arthritis. then i read the book by Dr Jean Seignalet and quit cereals (wheat and corn more particularly) plus milk and all dairy products. Miracle! one month later, no more acne, no more asthma, no more arthritis. It was in April 2003. Not a single pimple since then---except when i take wheat. Not a single fit of asthma or arthritis agains except when i have wheat for 2 or 3 consecutive days. I'd like to mention the case of a French football player, quite famous but I know nothing about these guys (I think it's Mossi). Anyway, his face was covered with acne... he got a liver-transplant that was successful. Right after the operation, all pimples disappeared and never came back again.
