Hello Dr. I was just diagnosed with 2 TI- Rads 5 nodules. Solid hypoechoic wider than tall smooth margins and punctate echogenic foci is demonstrated. Highly suspicious. This video was excellent it has helped me understand clearly about how the rating system works. Im so pleased thank you Sir! P.s Any tips on preparation for the FNA ? Have a blessed day.

Arrange for someone to accompany you on the day of the biopsy, wear comfortable clothing, discuss with your doctor if you should avoid supplements (if taking), discuss with your doctor if you can take a pain reliever before the procedure if you are concerned about discomfort. Thank you sir for detailed explanation. I am hypothyroid since 4+ years. Recently due to TSH fluctuation from normal to low did USG. It mentions heterogenecity with Colloid cyst in Left Lobe.TIRADS 2. Do i need to evaluate this further.

Most Welcome! Heterogeneity and a colloid cyst in the left lobe of the thyroid, with a TIRADS 2 classification, suggests that the features observed are typically benign. TIRADS 2 is considered benign with no further follow-up necessary specifically for the nodules in most cases. However, since you mentioned fluctuations in your TSH levels and have a history of hypothyroidism, it would be best to continue regular monitoring of your thyroid function tests and follow up with your endocrinologist. Quick Question: How often can a Thyroid Nodule's TI-RADS Score Decrease from a 7+ to a 2 w/in a Couple Months Time?? I had a Thyroid Ultrasound done a Few Months ago after Lumps started Protruding out of my Neck & was DXed w/ 2 Nodules both in the Left Lobe w/ 1st Located in the Posterior Upper Pole, which Scored a 7+ & the 2nd was in the Lower Pole, which Scored a 2. Dr was Concerned about the 1st Nodule, but not as much if the Nodule had been bigger than what it was at .5cm x .5cm x .5cm... However, I had a Follow up Ultrasound & Both Nodules came back as a 2. How likely is it the 1 Nodule went from a Highly Suspicious 7+ to a Not Suspicious 2 VS that they maybe Found a 3rd Nodule & Mistook it for the Suspicious 1st Nodule? I'm just REALLY Confused that the Score & Composition could Change so Radically. I've now had 2 Thyroid Ultrasounds, but the Dr now wants me to get a Head & Neck Soft Tissues Ultrasound. Thank you for any Light you can Shine on this!! 😁

Hello! It is unusual for this to occur. Here are some possibilities: Differences in ultrasound machines or operators could lead to different measurements and interpretations of the same nodule. It is possible although less likely for some nodules to undergo changes that could affect their appearance and score. It’s also possible that the initial and follow-up ultrasounds measured different nodules or that the highly suspicious nodule was misidentified as a different nodule in the follow-up scan. Your doctor recommending a head and neck soft tissues ultrasound will provide more details. Best Wishes!

I had a Head & Neck US of the Soft Tissues done on June 4th & they Found an Abnormally Large Mass/Node in/on the Upper Internal Jugular Chain of Lymph Nodes on my Right Side... It's stated that the Mass/Node is Abnormally Long. I can't Recall the Centimeter #, but I looked up the Conversion of the Centimeters & it Equals 1.5"... However, whereas the Thyroid Nodes were Found on the Left Side of my Thyroid, this Mass/Node was Found on the Right Side. The Radiologist that Read my Ultrasound said in my Report that I need a CTScan w/ Contrast to Rule out Benign & Malignant Causes, but that this Mass is Showing Signs of Malignancy. I also Received the PA State Law Act 112 Letter (It's a Law that was Passed in 2018 that Patients & their Drs HAVE TO BE Notified by Letter when Abnormal Findings are Found - I Received the Letter on Friday & it states I had an Abnormal Result & that it'll be Beneficial to Speak to my Dr & go over my Results ASAP. (I had a Head & Neck CTScan w/ Contrast done in Nov. 2023, when I got Cellulitis in my Cheek, that was used as a Comparison to this US & this Mass was not Present 7 Months ago)... However, since Nov 2023, I've had Cellulitis, then immediately got Covid (Dec-Jan), then immediately got Strep Throat (Jan-Feb), then immediately got Strep Throat again that turned into Scarlet Fever (Mar-Apr) & then got Strep Throat that turned into Scarlet Fever AGAIN (May-June)... So, I've been Sick LITERALLY for the Last 7 Months & Nobody has been able to figure out WHY &/or WHAT'S GOING ON , w/ Abnormal US Results, Semi Abnormal Blood Work Results, & just recently I Begged them to Test my T3 & it was a 4, while my T4 & TSH are at the Lower end of the "Normal" Reference Range... & I'm also dealing w/ Major Edema in my Feet/Ankles/Calves w/ Slight Fluid Retention throughout my Body (Which, I didn't Realize I had, until I took a Diuretic & I Shrunk! Besides my Lower Extremities going down in Size from HUGE back to Normal (I had Literal Cankles!!), so did my Hands/Wrists, Stomach that I thought was Bloat, but must've been Fluid, & Upper Body, bc I could also Breathe Better!! Idk what's going on w/ my Body, but Something is Clearly Wrong! I shouldn't be dealing w/ all of these Issues, Lumps, Masses, Fluid Retention at 41yo! I can only Hope that it's Figured out Sooner as Opposed to Later!!! Thank you again!!

I'm really sorry to hear about the ongoing health issues you're facing. It sounds like you've been through a lot! It's important to follow through with the recommended CT scan to get a clearer understanding of the mass in your lymph nodes. Please follow up with your doctor after the CT scan is done. It can also be beneficial to consult with an Endocrinologist for the thyroid issues and investigate any hormonal imbalances that might be contributing to your symptoms, including the fluid retention and changes in T3 levels. No, a T4 test cannot detect thyroid cancer. It is mainly used for conditions like hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid).

I am struggling to understand my TIRAD 3 score. My documents show a 1cm x .4cm x.8cm nodule on the right side. The composition is solid which tells me thats 2 points then the echogenicity is marked hypoechoic which should also be 2 points. Everything else scored a zero so would that not be a TI-RAD4? the report sent to my doctor rated it as a TI-RAD3 with no further testing recommended. The reason I was scanned was due to hoarseness and swallowing difficulties and now im afraid something is being missed.

Classification into TI-RADS categories can sometimes depend on additional subtle factors that may not be completely evident from the report summary you have. It's possible that some elements of the nodule’s appearance or other contextual patient factors influenced the radiologist's final assessment and classification. You can discuss your concerns with your doctor.

Possibly because i it measures 1.0 at its largest. Mine came back 1.7 on the right and 1.6 on the left, hypoechoic and was a TIRAD 3 also.

Though most thyroid nodules are benign, certain features (irregular margins, microcalcifications, significant vascularity, or rapid growth) raise concerns for serious issues. Usually they indicate benign conditions like hashimoto thyroiditis. Clinical and lab tests correlation is required for an accurate diagnosis. Best Wishes!

If a colloid cyst is anechoic, has smooth margins and consists of some comet-tail artifacts, it can be considered TIRADS 1

@@ПатяИсаева-ш2к Most Welcome! If a thyroid ultrasound shows a normal thyroid gland without any nodules, it typically does not receive a TIRADS score. TIRADS is specifically designed to score and categorize thyroid nodules, not normal thyroid tissue.
