I’m here for understanding. Best video I’ve seen. Had total thyroidectomy on 9/19/23. Got pathology results showing I have BRAF mutation. I’ll be having RAi treatment in 2 wks. I’m a 41 year old single mother. It’s incredibly difficult to plan for your possible death when I’m all my son has. So here I am, trying to educate myself and live longer

This was a very informative presentation on thyroid cancers. My mother has recently found a lump on her neck close to 2cm and currently being scheduled for ultrasound biopsy over at the LAC USC medical. I want to find a good institution for her so that she can be treated correctly from the start and promptly before the condition gets any worse.

I too am beginning this journey. Months ago I asked my young GP about taking a look at my thyroid and he asked me if I really wanted to go down that rabbit hole...made me feel stupid...now one of my lymph nodes is swollen and painful and I bullied him into an ultrasound. Lots of nodules that need aspiration. This was an excellent presentation!!!

I have cysts and nodules on mine. It first showed up on a mri of my neck after a car accident and i started off having one. I went a year later and i had 5. Now I've had several boiopes done and they do a thyroid ultrasound and a biopsy every 6 months, they're suppose to anyways. I shouldn't fall into any percentage of having cancer at all because we have alot of thyroid problems in our family . Thanks for sharing the video and the first biopsy wasn't painful at all but the second one was quite uncomfortable because i had more unfortunately show up. I need to have it treated asap and i see a surgeon soon because you re thyroid actually controls basically everything in you're body including your sleep patterns, you're mood swings, you're temperature and so much more

This is very informative presentation, I think more Surgeons should communicate with not just with the patient. I think more can be learned when the Surgeons, Endocrinologist, Radiologist, and Pathologist can communicate more effectively, share information, and most importantly go to Seminars using cross overs. A good example: You go into a seminar for an endocrine surgeon and you work as a pathologist, Why?! This seminar not only benefits the patient but, the pathologist can get a clearer picture what the endocrine surgeons see through their 👀 eyes, it give them another tool to help everyone involved. The same hold true for that same pathologist to go to seminars for oncology and endocrinology. I know this is a UCLA presentation: however, remember what I said about cross over?! The same holds true for Universities or other medical centers that are trying to help combat this cancer fight! I like a word that I heard by Dr. Julia Sosa- Endocrine Surgeon at Duke University medical center, she is also writes ATA guidelines, and is the president. She said: The patient must be part of the epi-center, this meaning the initial surgery is the most critical, and the best cure for thyroid cancer, the patient must make decisions as well within the ATA guidelines. I'll have to have a Lobedectomy and highly likely a full thyroidectomy, which for me is not only scary, but very challenging due to some lymph node involvement, and the current pandemic. The pandemic is hard enough, recently found out I am allergic to substances found in the shot, I've had already a serve shock that lasted for 3 days, was told I was lucky to be alive, what do I do??? Especially, they are encouraging cancer patience to get the shot, please help anyone??? I'm lost?

Excellent and informative. I had a total thyroidectomy and RAI back in 2009 (pappillary carcinoma). Given the advances in molecular testing and the newer guidelines, they might have gone with a lobe-ectomy and no RAI if i'd had it today instead of then, but better safe than sorry, so no regrets. QUESTION: Is there any data regarding the incidence of thyroid cancer that may have been caused by radioactivity affecting milk in certain regions ? I had none of the classic risk factors (i'm not in a low iodine region, nor was I exposures to known radiation sources), but I drank LOTS of milk as a kid back in the 60's & 70's ... cant help but wonder if there was any linkage between high altitude fallout plumes from secret nuclear testing at the time (re: cold war era).
@RovingPunster

@Wesley Hinson AFAIK, a fairly signifcant percentage of nodules turn out to be benign, and can come and come and go on their own. They can turn out to be just benign cysts, temporarily inflamed lymph nodes, et cetera ... but if a biopsy reveals a carcinoma, prompt intervention (surgery plus chemo and/or RAI) is usually indicated. I've had all three at one time or another. In my experience, homeopathy and/or medicinal herbs can be of some help for the first two, but tends to be a lot less effective with the third.

I got FNAC done 2 days back and report given is Follicular Neoplasm. The biopsy was done by locating the nodule with hand. Sonography was done separately a day before FNAC. Is it necessary to draw sample while detecting with sonography probe in parallel? If so, should I try for FNAC again at a different Lab to get a conclusive result?
@lexitejeda6209
@lexitejeda6209 7 жыл бұрын
Follicular neoplasm, from what I have read, is one portion of Category III or "Indeterminate" of the 6-tier Bethesda system of classification. According to journal articles I have found online, it carries a 15-30% risk of malignancy. If your cytology report says Follicular Neoplasm, you may want to have surgery to remove the lobe, or you may want to have a repeat biopsy, since about 80% of the repeats will give you a more definite answer, whether benign or malignant. If the nodule is too small to be sure they are placing the needle in the correct location, they will use ultrasound guidance. So no, it is not always necessary to use sonography for the biopsy.

Had a 3mm thyroid nodule that disappeared after a year of daily supplementation with lugols iodine, a couple of drops in filtered water each morning. I would certainly try a holistic approach with good nutrition and supplements before putting my fate in the hands of medical doctors, no matter how well meaning they are. Unfortunately, holistic medicine is frowned upon by the AMA because it hurts profits.

I had a what was a kind of benign slash malignant tumor. How that happens is the tumor calcifies and stops you from going through puberty till 40. Also it cause my tonsil surgery to bleed out in my throat because it was just hard and sharp enough to poke through my throat causing bleeding. The funny is I am growing shark teeth. I think my dentist attempted to kill the root and make the tooth dull but instead it rolled down my chin and took out the sharp thyroid tumor instead making my hair grow back so I now might have full head of hair.

Does your pathology report say anywhere the word "Indeterminate"? Or "atypia of undetermined significance"? I am not a medical professional, just an informed patient, but thyroid malignancy, if present, are typically slow-growing; so I would say you do not have to make a decision immediately, and take your time to study your situation. Get another opinion or two. Possibly have a repeat biopsy, especially if your was rated "Indeterminate".

Good presentation, I was one of the outliers, male, very physically fit had a nodule that was biopsied 2x and came back normal. It constanttly tickled my so I asked to have the nodule removed. They thought it was benign and surgery would only be a partial thyroidectomy and should take 2 hours or so. Surgery ended up being 6.5 hours long and was in 14 lymph nodes. I was 38 yrs old but history of fluoroscope use in my podiatric surgical work....

It is imperative you have a biopsy if your nodule is hypervascular? I really would rather try supplements first.......what do you folks think?
@lexitejeda6209
@lexitejeda6209 7 жыл бұрын
If it were I, I would have it biopsied by FNA. Vascularity is one of the features that makes it suspicious for malignancy. However, that is not a specific sign because more than 50% of hypervascular solid thyroid lesions are benign.

What are the guidelines as of 2019? My nodule has grown from 2cm to 2.1cm. I have multiple nodules front and back of right and left side of the thyroid. And yet to have more tests other than 2 ultrasounds. Thank you

I know someone in a similar situation to you. He has multiple nodules on both sides of the thyroid. He has so far had four ultrasound tests done over a period of several years. The three biopsies did not indicate cancer. He has a goiter which could be seen as a small protuberance on his neck. Similar to you, his nodule has grown and as such finds it difficult to swallow food. He used to feel as if his throat was constricted as if the slight lump inside was impeding the passage of food. His solution was to get the Triple Iodine complex 12.5mg capsule from Swanson Vitamins, break open the capsule, and take about one-third of the powder inside in the morning and with breakfast and the other third of the capsule after lunch. He says he could feel almost instant relief in his throat as if the lump has shrunk. He could now eat his food not fearing that it might choke him. He also feels that the aches and pains he had along his spine have disappeared. Iodine is required by almost every cell in the body for it to work properly. He feels that he has found a way to make up for this shortfall. Years ago they used to add iodine to bread. Now they add bromine instead which does not serve the body in any way. Another supplementation method is to use Lugo's 2 % solution on the skin. He prefers the Swanson Triple iodine complex over all others. The body needs Iodide and molecular iodine for it to satisfy bodily needs. Please do a google search to see how Iodine helps. Apparently, the vast majority of the US population is iodine deficient. please search Sherry Tenpenny, DO, Outlines many disorders that come with Iodine deficiency. Finally, I hope you get better.

@@saulbecerra5830 Thanks for asking. I had the thyroidectomy in december 2018. My TG did not go undetected. My lymph nodes were never tested. So I did finally have central left neck dissection this past July 2021. Right now, 4 months later, my TG is 0.06. Yay! I’m not sure Ill do Radioactive Iodine again. Only if my TG begins to climb. It’s been a journey.

my english is not good enough. please help me translate them into chinese. thank specimen: US FNA left thyroid nodule. clinical: left thyroid nodule. PRC:solitary 23mm left interpolar thyroid nodule. on today's examination this appears predominantly solid and markedly hypoechoic with possible micro calcification within it. macroscopic: six air-dried smears labelled left thyroid received. microscopic: the smears are moderately cellular and contain uniform thyroid epithelium occurring in microfollicles,flat sheets and as stripped nuclei. in two smears there are some scattered clusters of cells with more abundant dense cytoplasm and larger round to oval or occasionally pleomorphic nuclei.A rare mitotic figure is identified.A single atypical cell shows an intranuclear clearing raising the possibility of an inclusion. There is some thick colloid ,haemosiderin-laden macrophages and an occasional multinucleated giant cell in the background.There are no psammoma bodies. conclusion: US FNA left nodule: sugestive of a cystic neoplasm (atypical),a cystic lesion with a mixture of uniform thyroid follicular epithelial cells and atypical epithelium with some papillary like features (see comment). comment: 1. it is noted that an aspiration has been done on 26.09.16(Q16c28352x) that revealed a non-diagnostic results with predominantly cystic material only. the changes is considered. no cell block is submitted for ancillary testing.
