Real cartridges rarely find their way onto a film set, for obvious reasons. It's the fake ones that can kill. This type of accident almost always happens with revolvers. Unlike automatics, the bullets in a revolver can be seen in the chambers while looking closely from the front. So bullets (just the slugs, not a complete cartridge) are inserted in the chambers so that the revolver doesn't look empty during a close up shot, because that would be kind of cheesy; "how is he firing a gun that isn't loaded?". But since guns are meant to be fired in movies, there is the use of blanks. A cartridge without a bullet, to make a bang. Now, let's say you have an inexperienced, inattentive, or just incompetent armorer on set. And a blank is inserted in a chamber of a revolver that has a dummy bullet already in it. A propellant has just been combined with a projectile, and you effectively have a complete cartridge. Two props have now made a loaded gun.

I'm not saying it's impossible for her to have been high, but 20 minutes is a very short period of time to go from completely sober to baked enough to plow into a house hard enough to collapse a wall and set it on fire. The whole thing just doesn't make sense to me.

I had a dream about Jon Erik Hexum's death about 20 years after it happened, and I quickly realized it was my subconscious warning me to use more caution in a decision I was mulling over. And I did just that, abandoning a plan that it turned out involved more danger to me than I had realized. And Hexum was a person I had not even heard of until his death, but the news that he had died shooting a blank pistol at his head for kicks without realizing that can be fatal struck me as gruesome and bizarre. I barely thought about the incident over the decades ahead until my subconscious used his demise as a metaphor for the danger of not realizing what you're playing with.
