Adenomyosis is not an easy condition to diagnose. Adenomyosis has a lot of common symptoms with uterine fibroids and endometriosis, and it is hard to identify on an ultrasound.
Dr. John C. Lipman of Atlanta Fibroid Center regularly sees patients diagnosed with uterine fibroids, when in reality, they suffer from adenomyosis.

I have just been diagnosed with adenomyosis. I'm post menopause on hrt patches and had a bleed. Not heavy, no pain. Was sent for an ultrasound scan (internal) and this is the result. I am continuing with my hrt as I don't wish to have a Mirena IUS fitted.

I feel like this diagnosis was withheld from me. I should have had my hysterectomy at 13, and because gynecology as a whole won’t offer hysterectomy as treatment to women under 35, I lived with pain and the threat of bleeding to death, for decades. And now I have scar tissue and adhesions from all the unnecessary exploratory surgeries and ectopic. They literally tell us we are exaggerating pain, or can’t make a decision about fertility, because our crazy lady brains might regret not having children.🙃

Ultrasound said I have adenomyosis whereas in l.a MRI everything came out normal

That's very strange. Very often the low resolution ultrasound misses adenomyosis completely while the much higher resolution MRI is the gold standard to diagnose adenomyosis.

I had an intravaginal ultrasound and dr said I have adnomyosis & my uterus is 14mm in Luetal phase. For many many years I had SEVERE period pains and was just told it was normal. Now I am 46 yrs old. I must say I really don't have the severe pain or much for bleeding anymore like I use to as I got older it seemed to get better. I usually get my period 25-28 days apart but most recently I have had one period on day 32 and then 17days later (which would have been my normal time had I not gone to 32 days). In addition my Dr said my FSH says premenopausal but he thinks I need a biopsy of my uterus and an eblation or surgery. I'm not sure what to do because other than the andeomyosis my ultrasound was normal. I actually feel physically fine after all my life of severe pain during menses. I only went to the Dr for my regular pap. Do you have any advice or suggestions? Does this condition lead to cancer? Does it go away after menapause

Ablation should not be performed ever for adenomyosis as it typically makes the symptoms of heavy bleeding and pain worse ! Embolization or hysterectomy are the only two options that have been shown to be effective in the majority of patients.

Biopsy is fine. Ablation is not indicated for adenomyosis and typically makes the bleeding worse (analogous to peeling off a long scab uncovering a raw bleeding surface). Uterine artery embolization (UAE) is an excellent treatment option. The only other treatment option is hysterectomy and that should be a last resort option.

Hi, here in this report they say the uterus 5.9*4.0*4.5 is small in size anteveryed ET 2mm but also mentioned Adenomyosis

Is it possible to for a 57 year old post menopausal woman to have Adenomyosis and Endometrial Hyperplasia with Atypia ? ...20 day menusus occurred after 4 years of no mensus. Ultrasound showed 20 mm endometrial stripe.

Is CA125 level a good indicator to differentiate fibroids and adeno?

No. CA-125 is essentially of no value for women. There are too many false positives and false negatives. A lot of women's CA-125 level is elevated with fibroids (false +) and women can have advanced stage IV ovarian cancer and have normal CA-125 (false -). Fibroids are best evaluated with imaging. It's often an ultrasound. That's because almost every Gyn has one in their office, but pelvic MRI has much better resolution. Ultrasound often misses adenomyosis entirely and it is easily diagnosed on MRI.

sportsmd85 But I’ve done two different ultrasound. One says it’s fibroid while the other one says it’s diffused adeno. Is MRI the only way to nail it down? CA125 level at 190.

@@beznahmee3858 Yes. MRI will likely be conclusive. Sometimes adenomyosis can be focal and look like a fibroid but even then it should be distinguishable from a fibroid.

I had polyps removal but the doctor found I have adenomyosis. At first, she did ultrasound then she found out I have polyps. When she removed my polyps, she found out I have adenomyosis. Is it accurate her findings without using MRI?

Most likely so. MRI is the gold standard for diagnosing adenomyosis but it can be made on ultrasound. I am guessing you had symptoms and your doctor thought it was due to polyps. Once these were removed, if you were still having symptoms, she would need to explain why this was. She then discovered you had adenomyosis. There are only 2 effective treatment options for adenomyosis: hysterectomy or Uterine Artery Embolization (UAE). I believe UAE should always be the first option as it is safer, less invasive, less expensive and does not cause the woman to lose her uterus (which can have very significant negative consequences). Having low iron is often the result of heavy menstrual bleeding which is most commonly caused by uterine fibroids. The symptoms that result from having a low iron include: fatigue, weakness, lightheadedness/dizziness, brain fog (cloudy thinking), chewing/craving ice, migraine-like headaches, thinning hair/hair loss. A bulky, enlarged uterus can be due to adenomyosis. A pelvic MRI is the best imaging study to get to determine if it is adenomyosis or not. Ultrasound often misses the diagnosis, but if you had an ultrasound that described your uterus as bulky/enlarged with heterogenous echotexture, likely it is due to adenomyosis. is it possible to be diagnosed with adenomyosis and endometriosis? I was diagnosed with both condition and currently treating with Vissane medicine as per my OB advise.

Great question. Yes. Endometriosis is when the lining cells (endometrium) are found outside the uterus. Adenomyosis is when the lining cells migrate deeper in to the muscle of the uterus (i.e. just below the lining). Adenomyosis is in a sense endometriosis of the uterus. They often co-exist.

Yes, you can. However, Adenomyosis, like fibroids can also cause infertility. Gabrielle Union had infertility due to adenomyosis.
