Do all programs and I/O devices have the same virtual address space and different page table entries?

No, at least if I understood your question. Each I/O device gets it's own physical range of addresses. Each process (i.e., instance of a program) gets its own page table. But usually, as I cover in the next lecture, the actual operations to and from the I/Os are done using system calls to the operating system. So the operating system handles these.
