Sir ,this patient might have had intestinal obstruction 4 year back,in this we don't have any knowledge about that incident,they had resected that area partially during that time and this time patient might be having the recurrence of same illness

Why antibiotics?

Intestinal part proximal to obstructive becomes paralyzed, distended with stasis of contents from stomach/pancreas/Saliva . Edema increases , resorption decreases . Sequestration of fluid from circulation into the lumen leading to multiplication of gram negative organisms and anaerobes. Hence the need of antibiotics.
