They are used to it, a lot of animals including bears restrain themselves to their playmates level when playfighting, since it lengthens the time they can have fun and helps deepen their bonds. Usually it's while playing with another bear so there's not quite as much holding back, but the basics were there already.

For these reasons wrestling with a drugged bear, even if drugged by a professional, would be a higly risky if not borderline suicidal endeavour. Gaining it's trust to the point you can playfight with it takes longer but would be a lot safer. That doesn't mean I reject the possibility though since I have been on the internet long enough to know there's plenty of people who'll engage in higly risky if not borderline suicidal behaviour for cheap thrills or for a few clicks and likes.

Wow! I see people's hypocrisy here. Just because Alex Terrible is popular, they only hint that the Bear is being very careful to pretend some empathy for the animal, when they don't even know if it's been sedated or drugged, or what made it so docile. And obviously, someone's waiting with a dart just in case something happens. It's animal abuse any way you look at it, just to impress and for their own benefit.
