John could not fight the injection even in the slightest, even though he has taken down serious demons in the past (Gary's Mother, secret bosses...). The whole cutscene shows how powerful Gary's influence is, so that players truly get wary/anxious around Gary. And John?'s smile is a strong moment of this

In the cutscene after the injection you see the orphanage with 6 kids in the shot. 4 are gray and two are the exact colors for John and Lisa. 4 of the kids disappeared and were assumed dead and the two survivors were moved. Since Chapter 2 is all a dream for John, I'm thinking it was actually inspired by a memory. You also see a brief flash of them holding hands as kids, implying that they previously had a connection, which makes the Lisa ending make a lot more sense.
