This is very great lecture because we always see people rushing to psychiatric clinic or hospital before even examining the patient when it comes to altered mental state. i have seen this several times in psychiatric hospital where i am based since about 4 years now. Great video! I work as a Mental Health professional in a jail system. It can be difficult to determine the causes of AMS and this video offers a good concise explanation of the variables involved.

Yes, one hospital I was at put me on a psychiatric unit while I was basically slowly dying from an infection and dealt with physical altercations while hallucinating and being in and out of my mind. It took said hospital three whole days to figure out I had an infection and move me to isolation. The psychiatric meds that they put me on further made hallucinations from infection-based delirium even worse to where I couldn’t communicate properly. Since then other hospitals have treated me better when this happens with infections, but the first hospital left me with bad PTSD. Antibiotics do the trick every time to fix me.

A story I heard from a lecture was when a heat stroke patient (which also causes Altered Mental Status) we cooled down enough that he was back to normal. He then proposed to his fiancee afterwards. All the male healthcare members thought "ehhh, he's still altered".

My grandfather has been going through this because he has cirrhosis of the liver and he has to take lactulose to stop it.

I have hypoglycemia. It has not made me faint, but it sometimes makes me stupid. My liver does dump sugar, I believe, but super late, like 3 hours later. Blood tests say I am not diabetic.

Nothing like introducing POLITICS into the ER environment. Perhaps we need a seminar on Caregiver TDS ( Trump Derangement Syndrome) If you have ever altered mental status presenting in a patient, and struggle to diagnose and treat, you would not be so flip and unprofessional in your barb remarks.

I have to say this- not having a nurse or PCT to check a BGL is NOT a valid reason for not having the BGL. There is no reason a doctor, NP, or PA can’t perform a finger stick.
