My teacher worked in a nursing home with someone who's nickname was mad dog Murdoch. One day mad dog was feeding a little old lady her breakfast, but she was having trouble eating. He got about 10 spoonfuls in her mouth before he decided to go consult someone with more experience. Upon inspection this poor woman had died about 6 hours earlier.

I have a few: 1. Part time night auditor had not only slept through when the lobby was to be open (6 am), she didn’t have coffee ready, but also had infected one of the 2 front desk computers. But the real kicker was that her brother was sleeping on the couch in the half of the lobby that was exposed by windows. 2. In Army Advanced Training (AIT), guy was back talking drill sergeants and even instructors. His biggest issue, after the platoon had failed room inspections (I had gotten back from being hospitalized for a week, and my room was one of the few that passed. I even offered assistance to anyone else in my platoon), he has the nerve to say “My room wasn’t that bad”. The one drill sergeant proceeds to take all of us to his 2nd floor room, where pizza boxes, dirty laundry, and trash were strewn all over the living area of the room. Needless to say the guy got a dishonorable discharge (and he knew better, being the son of an Air Force commander). 3. Guy was more of a general areas cleaner, but had to mostly use golf carts to get around the property. He would drive them haphazardly and seemed to enjoy doing that. One time, he almost ran over a woman and her daughter, who then complained to the restaurant hostess for the property, who then called to corporate. The guy refused to apologize and thought they were just in the way. He got fired immediately.

I am in charge of the Capital Asset accounting where I work. A Sr Accountant tried to “help me” with a posting issue. He ignored my instruction sheet, ticked the box saying fully depreciate assets. He fully depreciated 24.9 million dollars of assets.

Story 4. I saw a chef do almost the same thing. He dropped a spoon/ladle into a large vat of boiling oil and without thinking he just made a grab for it. It wasnt until he had pulled out his hand did he start screaming. It was bad.

Yeah, sometimes you can get so used to working with potentially dangerous things that you almost forget it's dangerous... and then moments like these can happen where the part of your brain that warns you it's dangerous doesn't kick in until after the basic chain logic. (Dropped item, pick up item. While picking it up, brain says "oh wait, it's in dangerous thing!" Aaand you're now injured from reaching into the dangerous thing.)

The nightshift guy at the cloth factory is not completely at fault, why would you run a factory operation 24/7 with just one person in the night shift? That's bound to cause all sorts of problems. What if this guy had an accident and was all alone? What if something happened that stopped production that this guy couldn't solve? What about security? I can't, for the life of me think a valid reason why they would do this

Back when I worked security one of my nightly stops was at what could be considered a high end hotel for the area. They had a Night Auditor that only worked there for about 2 or three weeks before she was fired due to 2 major events: 1. There was some sort of youth event, I can’t remember if it was a school dance or some kind of church lock in, that was scheduled to end around 2 or 3am. This night auditor ended up closing the event out on the system around 6pm and I’m sure who ever ran the event was thankful for the discount. 2. The event that got her fired was pretty damn stupid. Some sketchy individuals came in and paid for a room with cash, had no IDs or credit cards, and completely trashed the room. The police had to be called because of all the drug paraphernalia left lying around and they had to test several substances found in the bathroom sink and on the walls which was later identified as Hair Dye. That auditor I believe was fired over the phone. At another hotel I ran security for one of the night Auditors came in on his night off with a small entourage, apparently they were celebrating his night off and decided to have a pool party. Dude wasn’t fired but his management warned him to never try that again.

I was working in retail as the ASM. Safe needed a battery change and instead of bothering the manager on his vacation, I decided to do it from memory. I remeberd it wrong. Locked us out of the safe for 3 days until someone from the company came. To quote the regional supervisor "well, people are always finding new ways to break things."

So I just got reminded of another one. The security company I worked for lost one of our sites, a saw mill, for a year after getting underbid by a competitor. Our company went through some hard times in the meantime which included lay offs until we got the site back out of nowhere and we even got bonuses. Initially we only got told the other company “took too many smoke breaks” until I met up with a friend who worked at that mill and he gave me the details. So for the set up the yard was littered with huge piles of bark left over from the process of prepping the wood, now I didn’t know this until I started working security but apparently large piles of bark like to randomly burst into flames so part of security’s job was to monitor the bark piles and call in a water truck from time to time to douse the bark piles. As I said before the other security company did have some legit complaints about the number of smoke breaks the other company took, my buddy said they had videos of the guards taking up to an hour at the smoking tables just chilling and after the complaints started rolling in they just started smoking in their cars. On one of these nights my buddy said you could see a guard sticking his arm out of his truck to toss something in the bark pile and then the pile itself “went up like Chernobyl” and that’s how we got our lumberyard back!

I watched a co-worker shoot his ex GF twice in the back, point a gun at me 3 times, he then shot himself, both died. This was in a prison with an open yard, and she had just relieved me, so I could go home for the day. The folks in charge never said a thing to anyone about them having issues.
