Hi Dr Prasad, @5:05 you mentioned about bone loss to the face. Does this happens uniformly throughout a person's skull, or it can be more in one place than another? Or does it vary in every individual? When you treat a patient, do you usually prefer treat the volume loss first? or treat the skin quality first? Thank you once again. :)

Hi, Facial bone volume loss is usually concentrated on the cheeks, jawline, and chin, so these are the primary areas of treatment. @@AmiyaPrasadMD Can you expand on your idea that there are limitations to how much collagen can be stimulated through in office treatments like RF and Laser? I saw a comment from you that the body’s ability to replenish collagen dips 1% every year, but that doesn’t seem like a meaningful amount to me. Seems like a nominal amount to me. Are you saying that for someone in their 40s and up they are actually getting diminishing returns on collagen stimulation?
@AmiyaPrasadMD2 жыл бұрын
Hi@@thad8909, Collagen in the skin is reduced by 1% every year, so with aging the skin loses its strength and elasticity, eventually resulting in skin laxity. While collagen induction treatments work, there isn't an unlimited amount of collagen you can induce the body to synthesize. Complications from these treatments usually happen when someone wants to avoid surgery, so they insist on stimulating more collagen to tighten, and lift the skin, but end up damaging the skin, and losing facial volume, so they end up looking older instead of younger, and could also suffer injuries like burns. For more information, you can refer to these videos about complications from collagen induction therapy, and how these treatments can be made safer: kzbin.info/www/bejne/q2exmJZ3n8qWfdE kzbin.info/www/bejne/kGbPqWZvYshjqac kzbin.info/www/bejne/oYvYq2aGpLmcZqs

Hi Donna Bowman, Tretinoin can help with cell turnover of the epidermis, and improve luster of the skin. Hi Amber Soots, I recommend 4 to 6 weeks between treatment sessions, as there is a limit to how much collagen can be induced. Collagen production is part of the healing process, so you need to leave adequate time between sessions for collagen to generate. Hi, Microneedling with a Dermapen is different from a dermaroller. A dermaroller unnecessarily stretches the microholes it creates with the rolling action, which the pen does not do. Also, dermarollers are usually used by people without formal training in microneedling, so they can be used too aggressively and too often, as compared with a professional with the pen, and giving adequate time for healing and collagen generation between treatment sessions. I hope this clarifies things for you.

Hi@@reemhejles, I also apply fractional CO2 laser for under eye wrinkles in lighter skinned patients, and often alternate this with PRP to reduce exposure to lasers and heat. As your doctor has examined your skin, and I have not, it's not my place to question their judgment, but fractional CO2 is a reasonable treatment for under eye wrinkles, so I wouldn't rule out the possibility for you. Hi Irene Delpiano, I don't recommend hot needles being placed into tissue at all, which can cook the dermis and subcutaneous fat. I don't think this type of device is as effective as it's promoted. I prefer limited use of more aggressive thermal devices like fractionated lasers, non-ablative lasers like Q Switch, milder radiofrequency like Pelleve, as well collagen treatments that don't use heat like PRP and skin boosting with hyaluronic acid. I hope this clarifies things for you.

@@AmiyaPrasadMD Hello skin Guru, Good day ! while in the process of body weight loss (or in diet) the face will get slim..at this time also will the face loss collagen ? if yes then how can we manage the collagen under eyes ? is it looks older while in diet ?
@AmiyaPrasadMD4 жыл бұрын
Hi@@kiranm7084, You won't likely lose collagen while on a diet, but you do need to be careful with your diet as collagen is synthesized by the body with food you eat. You will likely look older while you're on a diet because you will lose facial fat volume, and as people age, they do lose facial volume from loss of fat, bone, muscle, and soft tissue. Collagen does diminish with age as well, and we lose about 1% a year starting in your 20s. I hope this clarifies things for you.

@AmiyaPrasadMD5 жыл бұрын
Hi Jenna Steward, There is no specific treatment that is best for inducing collagen in everyone, as there are several treatments available depending on skin type, and the body does have a limit in how much collagen can be induced at one time. I do recommend alternating collagen induction treatments, say a laser in one session, then PRP the next, to prevent overexposure and overheating with laser devices. Hi Mary Wheeldon, Your body synthesizes collagen from all the food you eat. If you eat collagen, your body will still digest it, then synthesize it before it becomes collagen again. Dr Prasad, since you seem very straight forward in your approach I'd like to ask you what your thoughts are on a basically unspoken subject no plastic surgeon or surgeons in general will speak about. And, that is the FACT, as it's been proven, that surgery involving general anesthesia does sometimes, more often than not, more harm than good to a patient? There's much data, research and evidence that patients, especially later in life after 50 who undergo surgery involving anesthesia are more likely to establish cognitive impairment and ultimately Alzheimers and Dementia, as well as overall memory loss. As well, anesthesia to the overall body & mind is harmful. In fact, surgery can create a plethora of health problems against what we are trying to "fix" & avoid in having plastic surgery and that, among many things is an assault to the body, face and in turn creating free radical damage and an overall decline in health. I wish there were more HONEST Doctors out there that would speak up on this ongoing issue. People trust the medical community and look to you as healers, but the truth is the majority of doctors are more concerned with making money than the hippocratic oath they swore to "Do No Harm". Many reputable surgeons are starting to realize the harm in general anesthesia and are now taking a more - less evasive approach by using local anesthesia with sometimes iv sedation. I myself have had to go under general anesthesia many times and have personally noticed a HUGE decline in my memory and overall health. Please respond with your thoughts...

@AmiyaPrasadMD3 жыл бұрын
Hi Gen T, There needs to be enough heat for the body to recognize an injury before collagen is induced. I strongly advise you have an experienced doctor do this, and you don't try it at home with a device as you can end up making things worse without proper training. I hope this clarifies things for you.

@AmiyaPrasadMD5 жыл бұрын
Hi Ruma Walke, Thank you! I'm glad you found the video helpful! It is possible, but it depends on the area being treated, the issue being treated, and the extent of damage or aging. A physical exam is needed to determine the best treatment for you, and also to try the treatment to see how you respond. Collagen induction treatments usually take more than one treatment session for best results, so it is often a treat, observe, then treat again type of scenario. I hope this clarifies things for you.

I get you when you said how dietary collagen will be broken by our digestive system but i wonder why our cells wouldn’t use the protein molecules to make collagen for our skin to replace the ones we lose. So the phrase “we are what we eat” is kind of wrong. But shouldnt good things eg dietary vitamic C and anti-oxidants in fruits do our skin good? I thought people who take such things whether as supplements or in fresh food would result in better skin? Is this not scientifically proven, or is it just a myth or are the results hits and misses?
@AmiyaPrasadMD4 жыл бұрын
Hi Grateful Princess of God, The phrase is not necessarily wrong because while the body can synthesize collagen from a variety of foods and nutrients, the body does not synthesize vitamin C and antioxidants, which it does need to maintain health. I hope this clarifies things for you.
@wem-c9f4 жыл бұрын
@@AmiyaPrasadMD Good point, Dr. I wonder then why wouldn't our cells use the building blocks of digested collagen proteins we eat to rebuild collagen. I wonder if it's all coded in our DNA so as we age, we are not capable to do so anymore. We can do all that we can with modern science and money, but eventually there's nothing much left to do except to have sagging skin and bone volume loss and inevitable ageing. :/
@AmiyaPrasadMD4 жыл бұрын
Hi@@wem-c9f, Digested animal collagen can still be synthesized into collagen by the body, but other digested foods are also used by the body to synthesize collagen. Collagen in the body declines by about 1% a year after age 25, which is why treatments like lasers, radiofrequency, and PRP are used to stimulate additional collagen production as aging treatments. @AmiyaPrasadMD4 жыл бұрын
Hi tectonicblond, You are mistaken as I say that laser devices are just tools, and it is the practitioner using them that determines results, and complications like volume loss and skin thinning. With proper use and exposure, lasers can stimulate collagen, resurface the skin, and tighten the skin. With overheating and overexposure, lasers can thin the skin, and cook the fat layer below the skin, causing volume loss. Again, it is proper use of lasers by an experienced practitioner that improves your appearance; but overheating and overexposure by an inexperienced practitioner, or patients demanding a lifting effect to avoid surgery that leads to overheating and overexposure, that causes complications. I hope this clarifies things for you.
