The city was evacuated because of the monster attack. Superman can hear people from space literally. If there were truly people in theses buildings he would have not attacked Saitama here. And Saitama don't even pay a lot of attention for the civilians when he fight giants opponents. Just look at the way he beat that titan in the first episode of One punch man. Superman is only in the wrong, because instead of trying to calm the situation. He immediately attacked Saitama.

Saitama was damaged in canon before entirely breaking his limiter multiple times like vs Crablante pretty much every blue jumpsuit battle he took damage. Murata regularly ends chapters on moments Saitama seems like he might take damage only to be fine next chapter; it maintains tension in the story and one of the big questions is just how tough is limit broken Saitama. For example the dream Subterranean fight or the battle vs Carnage Kabuto.

Saitama never has a good fight(Literally the premise of the whole show), and therefore never gets hurt. His clothes are irrelevant, as is the fact he could be injured before he became One Punch Man. Having Saitama become injured for something like a gag with the cat or the mosquito is again humanizing, showing that he deals with the same things we do. He has never been injured in combat though because it would seriously detract from the credibility of the story as a whole. His limiter isn't faulty, it's broken entirely, gone, _whoosh._ As soon as that happened all his stats became infinite and everything we've seen has been an attempt to visually show us this infinite power in a way that's meaningful and understandable.
