Excellent overview. What wasn't mentioned is chronic fatigue, which is a primary complaint from patients who have otherwise been able to manage their illness with meds. None of the drugs to treat AS help much with fatigue. It's the most disabling aspect of my AS story, but it took 20 years after diagnosis before that started to seriously impair my function. Mentioning fibromyalgia without talking about disabling fatigue is a significant omission.

Yes! And it frustrates me when it's now chronic fatigue/long covid! Rubbish! I've had CFS for 20+ years. No covid back then.

@@ktcee803 The only possible upside of the Covid connection is that CFS is being taken more seriously and research dollars are going into studying it. Nothing will shut your life down more than CFS. It just sucks all the goodness out of it while you watch.

Almost 20 years I have suffered terribly and went undiagnosed. I have seen many rheumatologists and all said no to AS and possibly to psoriatic arthritis. I had a new hip at 40…not normal. Uvulitis, dactylitis, sacral pain. Still they were reluctant to diagnose. Finally I have found a wonderful rheumatologist who is treating me and I finally have some relief. What is the hang up with drs? I could have lived a much happier healthier and pain free life.

I'm a 72 Yo male, who likewise has had AS rulled out by Rheumatologist's exam, and a NEG HLA-b27 gene test. Sacroilitis in both SI joints. OA in both Knees, the right is Bone on Bone and will need replacement. My Spinal maladies have been diagnosed as Spinal Spondyliosis with Facet Joint Syndrome (facet joint arthritis), a number of sites of Mild to Moderate Stenosis in my entire spine, Cervical Instabilities and Loss of the Curve, and very thin spinal discs from Degenerative Disc Disease. I have suffered for decades since my early 30s. I fractured my right Femur in 1966 playing HS Football. The right leg is 1" short as a result. That Trauma has wreaked havoc with my body's physiatry - the leg length discrepancy was left Unaddressed by the Ortho Dr in 1966 when the Body cast came off. Despite the fact that there has ben no diagnoses of AS, the conditions I do have are very consistent with what has been explained here. SI Joint Inflammation is the Ground Zero for many Spainal issues. As my Rheumatologist explained - it all goes Up From There. There is no cure. I take many supplements to combat Inflammation and only ADvil / Tylenol to deal with the flair Up days. I got advice from a carpenter who had lots of srthritis : take 2 Advil in combo with 1 XTra Strength Tylenol - it works like a charm for me ! Advil is an NSAID, and Tylenol deals with Referred pains from the brain nervous system.

Chronic prostiatitis with AS, thank you for pointing this out. This is a huge symptom that nobody will address.

Sulfasalazine 500mg twice a day. And high sulfur foods.

Had a sports injury as a teen and was diagnosed with this and RA, however they did not perform that Schober Test and I went to the best local doctor which was Kaiser, I've been pretty good so far but every now and then excessive stress will irritate the injury, I wonder why they did not perform that test on me when I first went in

You need to see a rheumatologist! Ive just been diagnosed with AS after seeing one after years off useless doctors thinking I had back problems...

@@NathanChisholm041 I'm sorry to hear that, and I actually did for about a year at Kaiser, she said I had hairline fractures in my hips and the RA + AS was adding to it, but after doing my own research and tracking down the source of the pain I feel as if maybe they missed something, I'm planning to go in pretty soon here and get some x-rays and blood-work done hopefully I get some good news it has been 5+ years since the incident so hopefully it has healed

I've been eating very low carb, almost carnivore for a year and all my pain and other symptoms are gone. Still have pain sometimes from the damage done, but most days are great. No one wants to here it, but diet is EVERYTHING!!

It's only more rare in those populations which are underdiagnosed and misdiagnosed.

Im 70, HLAB 27+, female. For the last 6 months my right shoulder ligaments are slowly getting more painful. I usually get stuff on my right side..iritis, intercostal pain, knee pain etc. I'm taking antinflamatories.. any advice please help

Hello, my name is Prem Kumar and I am 44 years old. 27 years ago, I have suffered from Ankylosing spondylitis and I was a 17 year old teenager and I don't know the reason for my condition. It wasn't diagnosed properly back then but the effort to consult the doctors for the cure is huge. I have used many pain killers to keep the pain in check but it never worked out. A lot of pain is generated on my back, the neck and at the shoulder parts. And there isn't free movement of the neck and spine, I can literally feel the tightening of the muscles. After I had explained everything in detail, he said that there shall be a cure for these conditions that i was facing through meditation. So, from 8th November 2005, i was able to cure these health issues just through meditation, and by not using any kind of medication. Thats what my rheumatologist said, she said I have the symptoms of fibromyalgia BUT they're also symptoms of AS so she immediately ordered some blood work, a dozen or so x-rays from neck to pelvis, and a nerve conduction test because I have neuro symptoms in my arms. I have my follow up next week to get those results. I'm 39, but have struggled with low back and sacroiliac pain since my teens, getting worse over the last couple of years. Sciatica was a regular visitor, and now I have chronic muscle tension through my back, shoulders, hips and glutes, thighs, even my calves and shins, also gradually getting worse over the years. I'm hoping to finally get some answers, my brothers have suffered worse and for longer than me, but we're never able to get a diagnosis.

Hello I have AS been taking infusion monthly for about 3years but now I’m starting to gradually getting worse I’m hurting and getting stiffer even with the infusion. Could if stop working ? What will I do ?

Im right there with u i have had it sense i have been 15 ish according to my dr but was only diagnosed a little over a year ago in now 35 and each day is a struggle

Have just seen your comment - this is something that goes unchecked, but I have both, and the urologist could only reason that the prostatitis comes from periods of inflammation.

Some times i feel like am done with my life .... Initially effected my hip, then leg heel now my ribs ... I took 6 doses of exemptia , no relief ever....am just 28, will I live to atleast 50 😭

X rays every 3 months to monitor disease progress …not recommended per 2019 AS clinical guideline update

In patients with active or stable AS or nonradiographic axial SpA on any treatment, ACR conditionally recommends against obtaining repeat spine radiographs at regular intervals as a standard of care.

They can only be repeated, MRI nothing else MRI does not radiate at all and means, Nuclear Magnetic Resonance
