Blood sugar issues and high cholesterol is a consequence of this disorder (of starvation), not the cause.

So is the third symptom of not being able to tell how drastically underweight you are. It's a symptom of starvation. Not exclusive to the eating disorder. My mother was underweight after my father didn't allow her to eat (long story) but she told me later that at the time she didn't think she looked that bad.

Could some cases also be narcissists as they don’t seem to care about the dreadful distress they put their families under; are they actually’feeding’ off that distress to regulate their own dysregulated emotions.

Yes, yes, yes! Modern medicine and psychiatry love looking at all the dysfunctions of the human body as separate entities. It makes no sense to treat one area without understanding the system is all interdependent. Psychiatry is SO medieval in their understanding of abnormal behavior, trying to isolate a million symptoms and give each one a "special" classification. Narcissism is expressed in many ways, but the common denominator is CONTROL! ED's ......??? 💥 They all NEED to control as much as they can, are insatiable attention whores, love creating DRAMA, hate sharing the limelight, can't be taught, believe they are more deserving than anyone else, are rude and inconsiderate, manipulative and self-absorbed, have no guilt or conscience about the burden they choose to become, are "terminal" victims, refuse to take responsibility and will blame the dog groomer or their manicurist for their "pain." In my mind, I can't tell the difference!!

Anorexia is the opposite of narcissism. It's the core belief of not being worth food or life. The emotional issues are VERY often a result of trauma. Been dealing with restrictive eating disorders for over 50 years (started in childhood with being bribed to lose weight when I wasn't fat- I don't blame my mom, but I do wonder if I'd have been so eager to restrict when I left home for college if I hadn't had the "positive" reinforcement of getting paid to lose weight at age 7). I stay reclusive so I don't burden others- and any anorexic I've spoken to has been the same way. It's a dual way of thinking- the rational mind and the negative (ED) mind- and they are always at war. It's not a choice. It's hell. But it is treatable. I'm finally getting help from someone who knows what this is all about... and it's nowhere in the US.

To better understand what they grow up in, start with Sal Minuchin’s “Psychosomatic Families “.

Next minute this mental health issue will be regulated in terms of affirming their beliefs and parents will have to provide access to affirming services like liposuction.
