Just to clarify a small point: Clastin is not a beta lactamase inhibitor. Cilastatin is a chemical compound which inhibits the human enzyme dehydropeptidase. Dehydropeptidase is an enzyme found in the kidney and is responsible for degrading the antibiotic imipenem. Cilastatin can therefore be combined intravenously with imipenem in order to protect it from dehydropeptidase and prolong its antibacterial effect.(wikipedia). Very good lecture overall. Thank you

Tetracycline coverage isn't totally random, there is a general pattern: intracellular pathogens.

Great videos, Dr Bolin. Thank you for putting them up. Just a comment to try to improve this further: clavulanate and all the other compounds that are referred to in your talk as B-lactamases are actually B-lactamase inhibitors. The actual B-lactamases are enzymes that belong in the bacteria that we are targeting. These enzymes can inactivate molecules such as amoxicillin, and this is the reason why these inhibitors are given together with the active principles.

Very important additions: The fluoroquinolones also cover pseudomonas. And bactrim, clinda, and doxy all cover MRSA. Thanks for your presentation!

Excellent presentation. Vancomycin causes Red man syndrome. Aztreonam is not Aminoglycozide and is used in patient allergic to Penicillins. Vancomycin PO for C. Diff. Ampicillin or Vancomycin is used to cover enterococci. Thank you for your efforts

My Dr missed me on this. She said she didn’t know what she was doing. What she needs to treat. You tell me when you half a half dead parasitic die off, its excruciating. At least clean up after meaning in my body that was never going to happen. Not even my specialists referred to by her confirmed the irritation but didnt know how to treat it. I begged her. She dont understand the broad spectrum she shouldnt have her liscense.

Vanco is used po or by enema, not IV for C. Diff.

It is common to see Cipro as the DOC for Pseudomonas. I commonly use both Cipro and Zosyn depending on the situation. In the clinic I would give Cipro. In the hospital I would give Zosyn, Cipro or Fortaz. Any input?

minute 25, Dr Bolin is that true that imipenem is not effective against pseudomonas?

+ramez azzam Imipenem, Doripenem, and Meropenem are effective against Pseudomonas. Ertapenem is not.

No, it's not. That's why I have it in parentheses; I grouped it in with the AGs because they're both primarily used for gram negative bugs and I didn't have enough room to make a special category for aztreonam. I should have probably clarified that though. Thanks for pointing it out.

Paul Bolin Thanks Paul. I really appreciate your videos. After 13 years working as a psychiatrist I have now to prepare a like-USMLE exam in Switzerland. It's so harsh but I remain optimistic! You repeat that aztreonam is an aminoglycoside at time 31:11 of the video!
