What is not typically talked about in physical therapy? Documentation, Writing Notes, and Paperwork is not a common topic when it comes to PT. In this video I hope to break down the process of writing notes and how to document in physical therapy to help out PTAs and PTs in this career.

Hi Shelby! I recently graduated high school and started my PTA program a week ago. If you are able to seriously look into doing your pre requisites for dual credit. It has made my life much easier! I will be the youngest to ever graduate from my program now. Best of luck!

I definitely want to be a physical therapist when I’m older, so I love learning about this kind of stuff! I had no idea how much paperwork was involved. I hope it’s not too boring haha. Btw I just subscribed. Thanks for making these videos ^_^

I’m in PTA school now and I know this video is so old, but we are learning about documentation now and I was having trouble understanding the SOAP notes the way my professors taught us, so this video was so helpful!!

Antonio Sanson For the most part, yes! i’m only in my first semester so I know it’ll only get harder from here. I just take things one day at a time! But overall I love it, and I am always excited to learn!

I remember when I was a student, there is no day that We did not do SOAP notes every day for the 2years, Monday - Friday.

Thanks for the videos. I wanted your thoughts on insurance companies and their reimbursement rates for small private clinics. I'm finding that the reimbursement rates are not substantial enough to warrant establishing contracts with the insurance companies and are not open to negotiate the rates due to being a small business entity as opposed to larger Healthcare providers.

Hey Antonio loving the channel! Any advise on how to take better SOAP notes. I want to keep a good level of engagement (eye contact & active listening ) with a case study while documenting everything accurately. I have a hard time doing both at the same time.

Carlos Villanueva right I know what you mean, I feel like with practice you will get better at obtaining pertinent information to write in your notes, and you will get better at knowing what to ask the patient to meet what insurances want to see and things like that. We can try to put together a more in depth video in the future.

Awesome info. I always thought of specific notes as being most beneficial for the patient but I never thought about saving your own ass as well.

Just to let others out there know. If 60% is treating patients and 40% is documentation. That means 8 hours of patient work and 3.2 hours of paperwork spread throughout the day with most people skipping their lunch in addition to spending an hour after work to finish it, unpaid.

Awesome!!! A lot of jobs require note taking. I don't know if you mentioned it but, how do you keep track of what happened while treating the patient? Do you record the session and then take notes after? Because I know I'll tend to forget if I'm not writing as I go. But I assume you can't write as you go with physical therapy.

Miriam I. It depends on what works best for you. I typically will write my subjective at the start of treatment and then finish the other sections after the treatment session. But sometimes it’s helpful to write a few bullet points of important information in the middle of your treatment session while the patient is taking a small break and then go back later and finish your note.

Very informative. Just quick question, what sort of patient record keep software you use or recommend. And do you have all scanned for soft copies or you keep hard copies of patient records.

Great video! Does the notes have to be long? Do you normally have to put a lot of medical terminology, or can you use abbreviations in the notes? As a PTA can you give me an example on what you would put in the plan section :) You the BEST!

Anatomy depends on how much you do in the treatment section and then just capture the pertinent information in the note. I use medical terminology just to make my notes clear and simple never to add them just for the sake of having them. I put what I plan to do for future sessions. So if I plan to work on gait training and improving knee extension with stretching or something like that, then that is what I would put.

I have a question. I am currently taking prerequisites for a PTA program and it requires 2 math classes. How does math mix into physical therapy? I need to take elementary statistics so would I need to do some statistics while working or taking notes on the patients?

Erin Freeman honestly I don’t use math too often in physical therapy. The only times I can think of would be for counting your units you will bill out. But nothing too extensive.

How many hours/week do you think you put into schooling? I currently balance a 50 (sometimes more) hour work week, exercise at least 15 hours a week, and spend time with the wife a kid. I'm only asking because I am hearing from a lot of your videos that its time consuming although your version of time consuming may be different than mine. I want to be able to balance school , exercise, and time with my family. Obviously I can't work 50+ hours anymore. Thanks for all the information!

Nick Minor I’m actually gonna post a video next week on how to manage school and work! I don’t remember specifically but you do spend quite a bit of time dedicated towards school. As long as you can set up a schedule to designate specific time for school and to study then you can still manage to get some family time and exercise in as well. I believe in living a balanced life so taking time for each area in your life is important. But also remember that school is an important priority to finish because once you are done then you can have a more relaxed schedule!

Maricarmen F that is the section where you want to document how the patient is responding to the treatment. So you could document things that are important to how they do with the exercises, like things such as: if they are progressing, what are they struggling with, are they reaching goals, Things that would be important for how they are responding to the interventions and where they are still lacking. Does that help some?

Rahul chaudhari depends, Some people like to call themselves doctors or will introduce themselves as Dr. ______ . And they do technically have their doctorates degree but I usually just call them Physical Therapists not doctors. It depends who you talk to.

I think you’re sending out a mixed story here regarding time. So if you spend 60% of your day treating patients and 40% documenting, yet most rehab companies want at least 85-90% productivity….well, I think you see where I’m going with this…somewhere in that math you’re technically committing a no-no too because we can’t have you on the clock and documenting for 40% of the time. That would only make you 60% productive and that’s not good.

A noble profession, and much needed. But seems boring as hell! Spending 45 minutes trying to get grandma to raise her arm or lift her leg, and then writing pages of notes about it!...No thanks, but glad there are those who do this.
