Since all cortical impulses have to eventually pass through the axon (read subcortex), shouldn’t myelin/axon damage have the same effect as neuronal/cortical damage?

In some cases, it does. But often there will be multiple tracts and connections from one cortical area, so that can help differentiate if its a cortical or subcortical injury. And other specific signs of cortical damage like seizures.

Sir my father is suffered from ms from long time. This disease is making him weak time by time. He is on wheelchair from the last 5 years. There is no movements of hands also lately. It will be a great help if you could suggest any treatment or I could communicate with u on his situation
