Doc you need to know this.. My hometown is gorakhpur,UP. A tier 3 city. When I vsisited to my hometown after 3 years (PG prep and other stuff). Things were changed. Too many super specialists doctors. But most of them don't have command over their own subject. A DM gastro can't diagonse kidney stone from lab results and they went to Lucknow for such simple things. We have super specialists in tier 3 but they are mediocre level (not all but most) and people eventually go to Lucknow, delhi etc.. Even my father who had dengue was rushed to Lucknow. Doctors in gorakhpur couldn't handle dengue.. Lucknow doctors were laughing..

The most viable solution to increase the quality of specialist doctors is to remove any kind of reservation. An MBBS doctor is paid enough and is respected. Caste is no longer a barrier for any kind of social gathering.

Especially in surgical branches, there is no uniform training programs. Plus, MD/MS should be made a 4 or 5-year course

One of the reasons we don't get recognition in other countries is our degrees are three yrs, they should be 4-6 yrs and even we will have good recognition

I have a doubt sir, this concept of huge volume of patients are the only way to become good doctors exists only in third world countries. In developed nations, they get exposed to max 5-10 patients a week and its much lesser in surgical residency maybe one or two. Yet, they are producing world class doctors and we are nowhere near them. They follow everything from door to door from entry to exit. . I am surprised to see doctors who passed from premier institutes like AIIMS, SDPGI who are absolutely incompetent with huge list of complications while some from normal colleges doing fantastic work. There is no way to assure quality.

Of course no one wants to get treated by unskilled doctor. Reserved class is getting happy that they are getting seat...but in my city I have met with many cases in which patient cross confirm category of doctor and then goes. This change will come soon across India very quickly.
