I just had the ACDF C4-C6 two weeks ago. I have been watching these types of videos for the past week or so. I wanted to know what to expect within the first year after surgery. My surgery was successful so all the pain and discomfort I had been living with for years is gone. I am now dealing with the swallowing issue and just wanted to know more about the recovery process and time. This is by far the best video I have seen. You did an amazing job with the pictures you used and explaining everything so that it was so easy to understand. Thank you!

Happy to hear about your result, Dorothy. In general, patients will have some swallowing difficulty and some back of the neck pain (almost like whiplash) for around a month after surgery. This generally will improve with time. I do have patients that tell me that with certain foods and certain positions they "feel something in their throat" which may be some scar tissue between your esophagus and the plate or bones. The more time goes on the more that scar tissue breaks up. A very small amount of ppl have persistent swallowing issues (~4%).

I had a three level done six years ago. I had difficulty swallowing and talking for about four weeks. The pain felt like a combo of whiplash and strep throat, but the sore throat pain went away after a few days. The scar fades very nicely. It is barely noticeable now. I put scar gel on it once I got the okay from my surgeon and made sure to put sunblock on it. Good luck to you!

Had this done over 3 mo ago. Had 3,4 and 5,6 fused. The pain in my left arm is the worst I have ever had. I’m not a stranger to pain so I thought I could stand this pain but no it is so awful. Thank you for you info about it taking nerves a year to feel “normal”. I have nearly given up on being pain free.

Thank you for making this video. I had C4-C7 Disectomy and Fusion 1 week ago and of course I was expecting to come out with all my nerve pain gone. Nerve pain has been sporadic at this point and I forget how long I was dealing with it prior to the surgery. Your great explanation of what to expect after has set my mind at ease. I will be having my Post Op Dr visit Nov 7th and get an idea of how things are going. Again, thank you for making and posting this very informative video. :)

Thank you so much. I just had this done 1 month ago. I went in to the hospital for numbness in my arm and they explained I had a herniated disc and needed surgery and I got it done the next day. After watching your video I realized my drs left out a lot of information when they talked to me about it. I am a smoker and they knew that no one explained that that could be a risk of it not fusing told me not to take ibuprofen but didn't tell me why etc so I appreciate this video. At least now I know. Thank you.

In an urgent scenario, like it seems you are in, the benefits of acdf likely outweigh the risks that come along with not fusing if you are a smoker. The rates of fusion for a smoker vs. a non smoker for a 1 level ACDF is around 80ish% vs. 95+% so it isn't like a smoker will never fuse. Good luck!

This is likely in my future... neck pain and arm/hand numbness symptoms, MRI showed disc herniation. I'm a bit nervous about this and learning as much as I can. Your video was well done and you explained it for the layman... Thank you.

Hi Leah. How is your recovery? Have you noticed any limited range of motion with your neck? My surgery is 18 days away and I'm doing as much research to prepare myself.

@@bagofnothing make sure to follow your recovery instructions! I suggest getting a wedge pillow! And taking meds on a timed schedule, took about 5 full weeks to get range of motion back, but feel a lot better now!

Thanks for the info! It was very thorough and easy to follow. I had a horseback riding accident at 19 and now at 43 I'm looking at ACDF of C3-7 😬 I'm pretty confident that this procedure is my best bet. (I've done everything else already, eg. PT, injections, disc replacement, medications, massage, Thoracic Outlet 1st rib removal, etc.).

@@fritootr I'm 7 months post-op. I had ACDF of C5-6-7. It actually went really well! Took a good 6 weeks to feel somewhat normal but it was my 2nd surgery in the same area so swelling was a huge factor. But I'm 100% glad I did it. Because of my previous injury, the degeneration is progressing into my upper Cspine area. So will be dealing with that throughout my life. But glad I got the ACDF!

I just had my C4-C7 surgery on Tuesday. My pain level is 0-1. I thought I would be out for a while. I stayed overnight in the hospital. Nothing but the work of God! I'm uo moving around like no surgery was done. I did have swallowing day one,but that's it. Swallowing is fine now.

I had a cervical fusion 5, 6 and 7. Great video. Very easy to understand and follow. The one thing I'd caution is Tylenol, people tend to take that 4 months at a time to manage pain and recent research has shown that there are significant side effects with Tylenol in your liver.

Acetaminophen also depleats the brain of protective glutithione!Definitely need to be replacing that. It's horrible no one ever mentions that, especially for our growing children..

Hi, I had C6/7 surgery 3 weeks ago, didn't watch any videos, now in searching for answers this is the best one and the best explanation. Thank you. About Fusion - Does this mean that the two vertebrae become one big bone or is there only a small bone that connects them instead of disc? How it affects the mobility of the neck after 1 year? Can I go on waterslides, Yoga exercises stand on your head, bungee jumping and similar to requiring sudden movements?

Thank you for this video. It gives me a better understanding of what surgery I received. Had this done in Nov. 2021. It is now Jan. 2022 and it has made a world of difference for me. I no longer have daily migraines, I have use of my arms and my two outer fingers no longer hurt and I can move them. I have a decrease of neck pain by over 85%. Having the right neurosurgeon is a must along with his team. I live around St. Louis Missouri and have been suffering for many years and living on pain meds. Finally someone did something to help after years of suffering. Pain management just wants to give injections which did not work. I also was having other issues because of my nerves being so badly pinched. It is painful for about a week or two but I'm so happy now.

Thank you I have to have this in a few days. I had a fall and I woke up 2 weeks ago with numbness tinging weakness in my hands and arms first then legs causing me to fall. After several blood work tests, X-rays, ct scans, and mris , the nerve in my neck is severely pinched, and I must have this because I can barely move. I’ve been looking for videos to give me a better understanding because the Dr is a whole nother story. I’m 51 and my anxiety and panic has made this experience extra difficult but thank you

I had this surgery last week to repair three discs. Your video is so well done and informative and almost mirrors exactly the procedure described by my surgeon in Dallas. It helped reinforce that I made the right decision. The points about it being a marathon vs race also serve as a reminder why I’m not feeling like jumping into the Olympics anytime soon as well! Thanks for taking the time to provide this information.

May allah heal you , ihave the same discs as you gor injured from wrestling two years ago and theres no numbness or tingiling but crazy discomfort and blocked neck and shoulder and always try to crack it also my shoulders feels its not at its place . I dont know if surgery would make me regain my life because everyday i thunk of it and of the discomofort and i get tired easily.

I had ACDF on Tuesday morning, Sent home late Wednesday. Couldn't swallow Thursday, called the doctor, Couldn't swallow Friday. Spoke with the Dr. who siad it was inflamation. Could not swallow pain killers or muscle relaxants. Or a teaspoon of water. Could not reach Dr. weekend. Emergency Room early Monday morning - immediately Cat scan. Large Hematoma was completely blocking esophagus. 5 doctor meeting. 1)more Surgery, 2)radiology procedure with a stint - quite dangerous, or 3)steroids. Steroids was chosen I had NO water for five days and no food for 9 days. I lost 20 lbs in 9 days. First days of food was cup of jello and apple sauce. 5 weeks later I still have eat very small bites and slowly. I am a singer. I have no voice. Failure to monitor drain properly. 1% risk for this procedure.

@@Lord_Don Fantastic! No pain whatsoever. Every surgery is a little different, depending on some of the specifics of what they do. Some people just have herniations, etc. But I kinda had multiple problems, on multiple levels. Recovery time depends on a lot of things, age, complexity, the specific discs and vertebrae, etc. I think I could talk them into letting me go back to work. But I'd have to wear the collar, so I can't legally drive with it on. So just the logistics of driving me to work becomes difficult. And I would rather be careful and take the time to heal properly. This surgery is setting me up for the rest of my life.

@@Lord_DonLike I said, I think certain things play a part in length of recovery. I'm over 60 yo, so I expect my recovery is slower. Also, there are different cages that are used. Before surgery it sounded like recovery would be quicker than it is turning out to be, but I'm a patient patient. My only complaint is sleeping with the hard collar. I have a CPAP, so all of this hardware, tubes and and straps get difficult.

I had this done 21 years ago. Instead of a cage. They used a cadaver bone. And had to wear a electric collar to stimulate bone growth. .

Thanks for you incredible videos very informative. 6 years ago I had an L-spine surgery to remove debris and shave a bit of disc off which was causing severe pain and in some cases debilitating pain to left leg. After surgery 100% improvement. 2 months later I had MRI on C-spine which showed stenosis, bone spurs, bulging disc at c5/6 so a procedure was done from back and I saw some relief but not much. Six years later and I have many issues in both areas. MRI, CT Myelogram, EMG of C-spine show severe stenosis, myelopathy, ridiculopathy, foraminal issues, etc. My surgeon said he could fuse 3 to 4 vertebrae but I would hate him. I'm 56 and very active in the gym. I also have a white spot on my cord. He wants to be conservative and start with epidural steroid shot in c6/7 area. My left arm and hands are numb, I'm losing balance, not in pain but when I move I get shocks throughout my entire body. What are your thoughts? My MRI shows my cord looking like a meandering river!

Very difficult to say without a thorough exam and review. I would suggest watching my video on Cervical Myelopathy though and that can help explain more in general about your condition. The statistics behind those with cervical myelopathy (losing balance and shocks) is that people will progress in the severity of their symptoms about 2/3rds of the time but the speed at which they progress can vary from person to person.

I just had c4 through 6 done yesterday. Surgery at 7 am and was out by 3 in the afternoon. Slept laying down as normal and felt wonderful this morning (October 3rd 2024) many issues I had immediate relief from.

I will be having my surgery on 13.11.21. I have problem in c5 c6 c7(Disk herniation and compressed nerves...spinal stenosis may be).It will be anterior approach.My C6 will be removed completely.The disks between C5 and C7 will be removed.A cylindrical titanium cage(filled with crushed c6 bones) will be placed between c5 and c7 and a plate will be fixed with screws in c5 and c7. These are the symptoms that i am having pre surgery:- 1.Tingling sensation in left thumb and index finger. 2. Sometimes the left hand goes numb. 3. No pain in neck region and a rare pain in left shoulder. Now my question is:- 1.After surgery will i have unimaginable pain and numbness ? 2.How many days it will take to return to my normal life without any hassle anymore(pain or numbness)? 3.Will i have to live rest of my life in restricted way? 4.I love riding bikes and adventures...will it get stopped permanently?

This is by far the best presentation I’ve seen. I had C4-C7 in August. Recovery has been rough as I had lost function on the right side prior to surgery. Still have sensitivity in my right forearm but I can at least lift my arm.

Best to you. You and I have same issue. For me, i could not lift left arm. 1 epidural helped then 17 mos later c3-7 acdf but it was too late and spinal cord injury was perm. I did not really understand what cervical myelopathy meant and now advise everyone to NOT wait. My symptoms moved/ worsened b4 surgery on right hand. Am very lucky tho. Perm damage yes, but arranged for care at HSS. 5 days in hospital. Could not swallow water. Just take this yr 1 day at a time.

@@charlie0742001im much better. I have slight pain in my right arm still but nowhere near the pain I had before. I was able to start working out six months after the surgery. I continue to do low impact exercises. If you don’t have a recliner invest in one because I didn’t sleep in my bed for the first two months.

Well presented and great information. I had the ACDF done to my c4/c5 2 weeks ago and everything is fine except the muscle tension at the back of the neck and shoulders. There must be some kind of physio type of exercises that we should be doing after this to help?

Every surgeon is different. I typically like patients to "take it easy" for the first 6 weeks and then after the dust has settled from surgery, that stretch pain typically improves. If not I usually do PT at that point but every surgeon has their own protocol and one hasn't been proven to be better than any other. Good luck!

I had 3 level ACDF C 4 thru 7 3 weeks ago. I was driving after one week and pain free and overall much quicker recovery than I thought. Swallowing a little tough at first but all good now. My guidance is find yourself the best Neurosurgeon you can and ensure your getting up to date technology with Titanium cage spacers and NOT Allograft bones and plates.

Thank you, Todd. I use exclusively titanium cages as well. Glad you are doing better. I will also add that Orthopedic Spine surgeons (I am one) do the exact same surgeries are the neurosurgeons. I just encourage patients to get as many opinions as they feel appropriate for their condition and pick the surgeon they have the best rapport with. Trust is important when thinking about elective surgery. Good luck!

I had to have my c6-c7 fused last year so I’m coming up on the year mark now and I have to tell you I felt pretty ok right after the surgery. Now I’m actually scared because I’ve fallen a few times and broke my finger on my left hand and broke my wrist on the right. I’m also having an increase in pain from my neck down my arm. If this is how I’m going to feel for the rest I feel my life I honestly don’t know if I’ll be able to handle that. I tried to go back and tell the neurologist what was going on and had a mri scheduled but I lost my insurance because I was fired due to not being able to work. So I never got the mri done. I keep hearing on videos like this one that it’s an easy surgery and most people recover but I haven’t and I just wish the fall that caused all this would have just ended me last year so I wouldn’t have to feel like this all the time.

I’m afraid I don’t understand the “cage as a spacer”, I was aware of a bone graft (hip) and also aware of an artificial bone insert (cadaver), but I have not heard or been made aware of the “cage”. Can you elaborate a little bit more on what that is?

Sure thing. When you remove the disk between 2 bones there is a space left over. Usually under 5mm. You cannot leave that space empty because you removed the disk so you have to replace it with something. In an ACDF we use a synthetic cage made out of titanium usually that fits the left over space perfectly so it keeps it up and sometimes increases the space so it matches that of what it used to be prior to the disk degenerating. Most cage sizes are 6-8mm in height and we use the ones that best fit the patient's anatomy.

My wife (mid 60's and still employed) is having this procedure on Feb 3, 2022 and I just watched your presentation. She has been diagnosed with Severe Scoliosis but our surgeon agrees the higher priority at present is addressing the cervical spine issue. He plans to fuse C5-6-7 and place a cage between C6-7. He also quickly explained his "plan of attack" but your in-depth presentation made everything perfectly clear and answered all my questions. I so appreciate your very thorough addressing of Risks, Recovery, etc.! I am so glad I found your presentation. God Bless!

I had heard that when the cage is placed they put ground up bone taken from a rib or something and fill it to give it more to fuse to. Is this not done or only in special cases?

I was supposed to have C5-6 ACDF on Friday 4/1/22 but it was canceled due to me being a smoker. I was just about to get my IV inserted and the resident asked if I smoke, I said yes, he asked if I smoked that day and I said yes. He leaves and then comes back and tells me the surgery is canceled because I’m a smoker. I said “is this an April Fools Day Joke?!” … I was completely shocked and to say I was upset was an understatement. Not once was I informed that nicotine is a factor with spinal fusion, my chart says I’m a 15+/daily smoker and I just don’t understand how this happened. I’m still in pain and now I’m going cold turkey after smoking for 25 years. I’m also finding out that nicotine has very little impact to fusion for single level cervical ACDF with autograft which is the surgery I’m getting. I’m soooo unbelievably upset and I have no idea how long I have to wait now.

I'm very sorry you had that experience. Nicotine can have an effect on fusion rate historically but newer data has come out that is combating that narrative. In smokers, I have been doing disk replacements more often since in a disk replacement you do not want a fusion to occur. This has been a great option for those who are candidates of a disk replacement who are smokers because they can get the pain relief and we don't have to worry about issues healing.

I had an ACDF AND I regret it entirely. My doctor didn’t even tell me disc replacement was an option . I’ve competed in four Olympic trials, and my doctor didn’t even mention disc replacement . Acted like fusion was the only option didn’t seem to care about my desire to coach. I feel I was robbed of my mobility. It gets worse every month. Basic exercise seems to make me worse and that’s not right. Can an ACDF be reversed with disc replacement? Any advice would be amazing

As a candidate for this surgery, I have recently read and listen to a lot of spinal fusion experts/neurologist talk about how implants are not bone friendly compared to allograft and of course, autograft. With many recent studies discussing the high rate of nonunion of cages in ACDF, why do surgeons continue to use them if other methods are available, especially given the steep consequences.

Excellent question. We worry about fusion which has been shown to be decreased in smokers and patients with soft bone. Regarding swallowing difficulty after surgery, odds are increased with age, multiple levels done, higher levels in the neck as opposed to lower levels, and history of prior swallowing difficulties.

I am scared because this area of the neck has many nerves around it. Should I seek a second or third opinion on the MRI before making my decision, Doctor? For moderate to severe cervical stenosis, ( C3-5) what would the advantages/ disadvantages be to a posterior laminectomy vs. an ACDF? Why would a surgeon choose one over the other? The reason I am asking, is that I have had consults with 2 orthopedist spine specialist surgeons, and both explained their opinions convincingly. I don't know what to decide... I am considering the demeanor, experience, etc. of each doctor of course, but most important is what type of surgery do I need for my particular situation? What should I research? How do I know this?

Hi Annette. Speaking generally, anteriorly based surgeries have quicker recovery time and are less painful but sometimes there are factors that push surgeons to try posteriorly based procedures because the anteriorly based ones may pose more risk. Surgeons will almost always choose an anterior based surgery compared to a posterior based surgery unless there are extenuating circumstances. Speak with your surgeon and go with the one who you feel most comfortable with about treatment options. Good luck!
