Good. I don't know how many times I've done this procedure but it must be in the high hundreds. I use an iodine prep. and very much agree with the injection of a small quantity of local anesthetic first. I use an insulin or tuberculin syringe and needle with lidocaine It makes the procedure so much more comfortable for the patient. I also puncture the radial artery a little more distally where it is slightly more superficial and a little less mobile.

why did you expel the heparin from the syringe? i thought you needed a little heparin in the syringe to prevent the blood from clotting?

Thanks for this! I’m a neuro-nurse and we’re allowed to take ABGs at my hospital (in Sweden). However, no one at my unit seems to do the Allan’s test - is that a must in the procedure?

Charlie no, it has poor sensitivy/specificity and even if positive, then what? Should ABG not be taken? The indication for the ABG in an emergent setting is almost always high enough that one should not even consider Allens test, in my opinion. Best, Abraham, em doc, sweden

I'm confused, are we supposed to expel the heparin or leave it in the syringe when we take abg? Thank you.

Definitely not. And afterwards you're supposed to the roll the syringe between your fingers to mix the blood and heparin together, and make sure that all air bubbles are expelled. Not sure why he is saying to expel the heparin in this video.

Heparin prevents the blood from clotting. Not sure why he's expelling it.

As long as you coat the barrel with heparin, it's ok to expel some. Too much heparin can result in a bad blood gas...

Usually the syringe has heparin but the syringe should still be rolled to ensure no coagulation occurs while bringing it to be tested . At least that's what we've been taught

I do arterial puncture in rare cases of difficult venipuncture outside the context of ABG test. It scares me a lot and my hands tend to be shaky when i resort to this. Nightmare.

In hospital settings, we don't use lidocaine at all. We simply stick. The initial poke will sting a little bit but the real pain is if the needle accidentally hits a nerve.

I've never not done the Allen's test. And I've actually refused to procure an ABG on a patient who did not have sufficient ulnar circulation.

Well. No. Because you are going into an artery and not a vein. But you can have bruising and soreness at the site.
