Omg I'm so mad at the facility my aunt is staying at, they are not using her abduction pillow and cause her to have joint dislocation , her leg that had the hip surgery is tilting inward. 😡😡😡😡 and now she is acting strangely and has a BP of 87/62 I think she has an infection and they didn't keep her surgical site clean and dry. She has a foley catheter and I feel they weren't doing good foley care on her. She was transferred to from a hospital to a snf and this is what happened. I think they should send her out back to the hospital to get the care she needs, this place is for the birds. 😡💢 I just don't want her to develop sepsis from their poor care. I'm a new grad RN, any advice I would love to here from you, please. 🙏🙏

I would call 911 and have her taken to the ER. A VERY low BP and new onset confusion is suspect for sepsis… she needs to be assessed. Sending healing thoughts to your aunt. ❤️

@@LevelUpRN Hi thank you Cathy for your quick reply! So I went do to see her and assessed her. Her heart rate was normal, in the 80s, but her BP was low, like 87/68. She is on Rocephin for 3 days and is getting IV fluids. I did see the fluids were dripping super slow (it was via gravity) I kept asking about it and they finally went into check it, to find out her vein was inflamed since the iv push was causing her pain, they took out the IV and placed a new one and the IV fluids were flowing better. I'm so glad I caught that. And I made them make sure she was wearing her abduction wedge in bed since her surgical leg was starting to turn inward. I'm just glad it wasn't sepsis. She does have a foley catheter on so my guess it was a foley catheter infection or she was just really dehydrated. But yes, I would have said for them to send her out to the hospital ASAP if I knew it was sepsis. Anyway, again, thank you Cathy for everything and being so compassionate to a fan in need. 🙏💕☺️ P.S. She's doing much better now, she sounds more with it and the color came back to her face. Iam glad her surgical incision site has been kept clean and dry. I had to help the CNAs clean her foley when I was there. I felt like I was working a full shift, I was pooped afterwards. I just wanted to make sure she was taken care of. I'm glad her doctor was on my side to make sure she was compliant with PT and OT (I found out she is refusing) and I encouraged her to drink lots of water to flush out that bacteria. Her urine was amber, now it's yellow. ☺️

Hey, Cathy! I've been reading that antibiotics before dental procedures are no longer recommended in individuals with TKA or THA.I was wondering if you had read anything about that. Thanks!

Hi there! You are right. New ADA guidelines indicate that prophylactic antibiotics are only needed in a small percentage of patients with implants. However, a recent survey of orthopedic surgeons indicated that about 75% still recommended antibiotics prior to dental work or other invasive surgeries.
