What does it mean when sTfR is too low with a high ferritin?

When sTfR is too low with a high ferritin level, it may indicate that there is enough iron available for red blood cell production, but the body is not effectively using the iron to make new red blood cells. This situation can occur in conditions such as chronic inflammatory disease, infection, malignancy, and some types of anemia. In these conditions, the body's immune response triggers the release of inflammatory cytokines, which interfere with the uptake of iron by red blood cell precursors. As a result, there may be enough iron in the body, but it is not being utilized efficiently for erythropoiesis.
