Actually #4 she is signing (initialized I" sign for) Indianapolis, (fs for) Indiana. That's where Riley's children hospital is located.

Yes, that sign is for Indianapolis, and the lip movement is for Indianapolis (in Indiana) also. I believe people sign "IND" for Indiana.
