Here in the USA, at least, psychiatrists must all go to medical school, so the ones here are medical doctors (either MD or DO degree). Of course, the DC Universe is fictional, so perhaps Harley was able to get her job treating the Joker without going through medical school first. Or maybe she just forgot how to do a basic exam because medical school was so long ago for her.
