It wasn't accidental, he very clearly did it on purpose. If it had been accidental, then why would the asshole have acted dumb by refusing to lift his foot when told to (if he had done so and the coin wasn't under his foot, then he could very easily have turned the whole thing back on the kid and made himself look like he was the victim instead of the bully that he was). If it was actually accidental, he would have lifted his foot to see what the problem was in the first place. The guy was being a total dirtbag by not only pushing that old lady to the ground, but also trying to cheat her out of some of what little money she had. And the other people here were no better since nobody else even bothered to try and interfere or help the old lady out when all she wanted was a single apple.

For the time period, I think his reaction is quite normal. She was also declined at first but still decided to try and beg some more. Whether or not he intentionally put his foot on the coin is subject to interpretation. If you tell someone to lift their foot up, you should give them a reason to... But our MC didn't, just jumped the dude for asking. The whole situation was just clichè though and it was done pretty poorly.
