I'm pretty sure the ending is showing us Mikasa passing on to the afterlife right after she died from the old age. In the beginning we saw her jump into the water and then floating in it surrounded by white cherry blossoms which are often used in funerals in Japan and represent the transience of life, rebirth and a new beginning. We see her floating in the water surrounded by them in the exact same position she was buried in, which signals her passing away and her journey into the afterlife. We then see Mikasa experience a dream state where she sees birds on a fence and right under it we are shown 4 red roses which symbolize "Nothing Will Come Between Us", one of the birds is holding 1 red rose signifying "you're the one and only love". Then we see her and Eren running around being happy and at peace together, leaning into each other and being there for each other. She then holds 2 yellow chrysanthemums shining bright which symbolizes the long life she lived with both sorrow and neglected love in her heart and her closing her hands putting out its light showing that its the moment of her death. The final lyric of the song is "I know I'll see you again, lets say goodnight" as Mikasa opens her eyes in the exact same position as she was buried. Her eyes full of tears thinking its just another dream again but no, she has a surprised expression because she sees Eren is right there with her, holding her hand with a yellow butterfly on his hand which represents rebirth, they're finally reunited in the afterlife free from war and cruelty of this world, together for eternity 😭

You're absolutely right, and to some people the afterlife concept seems vague, and they just say that it's fan service. But at the start, we see paths, which indirectly might mean that they reunite in the paths, and not what we refer to as afterlife. In short, just like Ymir, who still remained in paths after death. So I think Eren and Mikasa reunite in the paths, The afterlife thing is making others say it's fan service lol, but it actually had to happen at some point due to paths. And thank you for the symbolism :)

I refer to it as afterlife because my assumption is that there's no "hell" or "heaven" in AOT but simply a place where your soul can finally move on and be free with your loved ones. I'm making that assumption based on the fact that we saw Hange reunite with other dead survey corps members right after she died and we saw the "ghosts" of Sasha, Hange, Erwin and others appearing right in front of other living people to have their last proper goodbye between each other. Which in retrospect is interesting because right after that we see Mikasa meet Ymir the same exact way before she disappears herself. So yeah, for all we know the "afterlife" of AoT world are actually paths your soul moves on to.

I think the ending of the song really showed Eren and Mikasa to finally be reunited in the afterlife. I know this might be a stretch and you may think I pumped myself full of copium but please check this out. I'll explain the video animation, the lyrics, the symbolisms, foreshadowings and such. Remember, Isayama always added symbolism and foreshadowing all throughout AOT by using visuals of flowers and the like and it shouldn’t be a surprise that he did so even in the animations of the final song of the series. Illacertus First, let's talk about the title of the song- Itterasshai. This was of course a callback to the first episode and to when she was saying her goodbye to him but this could also mean other things. For example it could also mean "Take care.", "I wish you luck", "Have a good trip", "Let's see each other again" or "Be safe" Anyways. At the start, Mikasa is seen longingly gazing at the moon, and she's 'submerged or wading through still waters' obviously because of the loneliness that comes with the loss of her beloved. Eren was her rock, her hope, the only real family she had left. Then we see a brief glimpse of the Paths followed by growing white plum blossoms. This symbolizes Mikasa grieving but then she remembers all of Eren’s memories telling her to be strong, and to continue living and to die happy. Just like how the song said: "Yatto ima te ga todoita Watashi no mune ni mimi o atete, Anata wa zutto koko ni iro/ But now at last, you are right here in my arms. Rest your head, and put your ear upon my chest. Know that you have always been here with me." She knows he's always with her and she hopes about the happiness to come. For example, he appeared in front of her as a bird, why can't he do that again, right? So she's confident he's always watching her. Edit: The white plants could also be Lilies which could mean a lot of things since she was laid down on her deathbed filled with them. White Lilies symbolize purity, commitment and virginity. Mikasa stayed pure to the very end and was committed to her one true love. If we take it literally, This could mean she was undergoing the process of rebirth when she was seen in the still lake with white lilies floating in the water and she finished when she was submerged in it. The next scene comes and we see Mikasa looking at a flock of black tipped doves, focusing on the one with a rose on its beak. She keeps watching it even as it flew past her as she remembers herself and Eren in their childhood wandering through the fields when they were children; innocent and young. This signifies Eren’s spirit, memories, and their love that endures even in his death. Reinforcing my first point. It could also be that Eren wants to show her that they're free now. The Titans are gone, the wall is gone, and even if it could not have lasted forever, at least in Mikasa's lifetime, the threat of war is gone. She doesn't need to worry about him anymore and he wanted her to just forget about him. (He's lying.) Like he said, he wanted her to move on with her life (even though he knows he doesn't mean it. He parted that with Armin and how he reacted all the way back in Episode 1 when everything started again). Fortunately, Mikasa couldn't let him go. That's why she keeps looking at the bird that symbolizes him. Her eyes still look for him in everywhere she goes. When given freedom, all she wanted is to bring back the home that allowed her to fly freely. The chorus lyrics then sings of how she wants to do what lovers do with him. How she longs to talk about love with him, how she wants to be with him, how she wants to grow flowers with him. That had always been a passion of her. How she wants to play with him again, laugh with him, be sad with him and cry with him. They'd get lost together in the forest and she'll protect him, comfort him and hold him all throughout the way. She just wanted to be together with him. In the scene as well, Mikasa was walking with Eren on a peaceful afternoon field teeming with flowers. (The flowers i can see were Camellias, Marigolds and Daisies) She the picks up Marigold flowers on her hands. Marigolds symbolize strength, light, and hope. It also signifies sadness due to losing a loved one which exactly describes everything that Mikasa is feeling. In the fourth scene, Eren and Mikasa as children are both playing under the shade of (presumably) his favorite tree, The tree we saw at the first episode and where he was buried in the last. This was also where Mikasa asked to be buried in to be with him even in death. This either means she was reminiscing about their childhood and this could be what she wants them to do again. Edit: When taken literally, this shows them when they reunited in the afterlife. And finally Mikasa wakes up with tears in her eyes as she was sleeping in a grass field filled with White Chrysanthemums and White Daisies. She then looks to her right to see Eren’s hand gently holding her own, a butterfly placed atop of it, flying away shortly after. Now here’s the reveal: White Chrysanthemums, White Daisies and that butterfly, specifically, Japanese Yellow Swallowtail butterfly are all closely related with death, reincarnation, rebirth, the afterlife, new beginnings, renewal and meeting your lover again! It's literally telling us that they are now given a new life through something like reincarnation or rebirth! This is when she awakened to finally reunite with him in Heaven! This whole song is just a message from Mikasa to Eren. All her thoughts to him resounding within throughout her life. It's everything she wanted to say to him and more. Say for example, verse 2, "Zutto kidzuiteta tsuyogari darake no anata ga Tadoritsuita kotae no saki ni otona ni natteita koto/ I had alrеady known behind your facade that you're fine, that you're tough. Far beyond all the answers you've reached and found. You had grown into the man you are today." and the secind chorus: Tsukanda te o furiharatte Tsuyosa to kodoku o te ni totta Ookiku habataki sora no mukou Yatto ima te ga todoita Watashi no mune ni mimi o atete Anata wa zutto koko ni iru/ As you shook away the hand I held so tightly. You took instead, all the strength and all the loneliness (in yourself). Soaring up, so far beyond into the sky. Let's go back home, where we lived together." ALL OF THIS IS JUST A BEAUTIFUL RENDITION OF A TUNE ABOUT THE WORDS HIDDEN WITHIN, BEATING INSIDE MIKASA'S HEART! The words " Mata aeru yo ne? Oyasumi./ We'll see each other again soon, right? Goodnight.", is literally what she thought of every night before she dreams, what she thinks to Eren to strengthen herself enough to get up, and what she hoped would happen in her entire lifetime to finally happen. Maybe even her last words before dying! The first point that reinforces the theory is that the literal start of the song said, "I kept trying to find the place for the heart i gave up.". This was the question that could never be answered her whole life. Since the one supposed to answer is no longer in this world. But then, in the full song, it repeats the opening line and just answers its own question at the end (This was because she finally found the place of the heart she devoted when she met Eren in the afterlife. "I kept trying to find the place for the heart i gave up- Who would've thought, who would've guessed that i'd find it here, right beside your heart." Credits to: qwerty death's comment Edit: If interpreted in another way, it tells us that Eren is the only one who belongs in her heart forever. Another point is that this whole animation could be a jumbled order of events that took place after the two met in the afterlife. In the last scene, she wakes up to reunite with Eren. Then Eren helps Mikasa up as they take in the feeling of being together again. They run together like they always do as Eren guides Mikasa to (presumably) their home just like what Mikasa always wanted. To be with Grisha and Carla. And lastly they hold hands, finally in peace together. :Like what is said in this verse "Saigo ni nani ga shitai? Doko ni ikitai? Watashi wa ne kaeritai yo Issho no ie ni kaerou yo/ In the end, what do you want to do? Where do you want to go? If I could have my wish, I want to go home. Let's go back home, where we lived together." Edit: Wishes really do come true, huh? Edit 2: This could even be a montage of what they did in the afterlife. Such pure souls! (Also if you wonder why both of them are depicted as children is because it represents the innocence and purity of the two lovers. It's also because it represents the happiest moments of them together when they were alive. Back when everything was simple. They didn't know any better but they were complete. When they just lounged around and lived their lives. There was always the threat of Titans but they were happy. But now in the afterlife they don't have to worry about that anymore. And they're finally going home at last! What could possibly be a better ending for the love birds! To conclude, after we see Mikasa’s death in the show’s ending, she wakes up to find Eren holding her hand as they finally get their happy ending in the afterlife in this song and animation. They pulled a Titanic on us. What do you think? I’m sorry if my English is bad, this really isn’t my first language. Thanks for the explanation, I like it. I don't have a problem with her marrying Jean. If it couldn't be Eren, he seems a good fit, given their history and his care for both Mikasa and Eren. And, as she's buried with both her wedding ring and The Scarf, given that the dead don't clothe themselves, I think her enduring love for Eren (almost in the sense of a widow) wasn't a hindrance for her later relationships. A 'happysad' life, which, I have to recognize, is fairly realistic.
