Hi Dr Tim, for volume loss in forehead and frown lines, I have been advised to filler injected with canula for both areas. At the same time Would you concur with this approach?

Isn't it true that some thicker products do not aspirate. If a product is too thick you can't tell if you're in an artery while aspirating there is no flashback? Is deep filler the same thickness as hair gel?

Dr. Tim, when you say inject on the periosteum level for nasolabial folds, how do you do that without being inside the mouth?

Only in the piriform fossa, lateral to the alar. You are correct if you go down the nasolabial fold you will enter the buccal cavity. I recommend a cannula for more inferior product placement.

Suh great information. Thanks so much. What about threads. I often hear there is no chance of occlusion from the needles we use to insert pdo threads even though in a lot of instances we are parallel to an artery such as the vermillion border and the nasiolabial folds.

There's no real VO risk from threads. You are only in about 2mm & not placing anything with viscosity - it's a tiny thread that causes collagen and elastin production.

Is there any of his videos where he discusses how he caused a V. O when working from the back of a salon? Id love to see his thoughts looking back when apparently he mis diagnosed it & gave the patient a concealer pallette to cover the bruising.

Sigh. I think this is happening to me right now. Had an awful job done a month and a bit ago. It's very overfilled and has been burning since. Today my entire right side of my face is discolouring (the same side as that bit of filler that hurts) It's the middle of the night and it's only started showing now. Can't believe it.

Hi Tim...what do u think about the risk of skin boosters - restylane injections over the forehead...with the smart click ..is 0.01 ml enough to cause a vascular occlusion..? Do put ur views...

Some skin boosters contain cross linked and non cross linked ha. If there is cross linking there is always a risk of vascular occlusion no matter where it’s injected and with what ever device and what ever volume.

0.01mls is a very small volume though- even if it occluded some vessels it would be a very small injury imho. -Tim

there are several sizes of blood vessels, arteries for instance: large arteries (like the aorta), medium size arteries, arterioles, and capillaries. Okay, let's say a cannula breaks/damages an arteriole. What happens then to the tissue that was supplied blood by it? Can cannulas puncture venules and arterioles, or do they slide by them?

They can penetrate vessels, but they more often push them out the way. Needles are just multipliers of force per area, and cannulas do the opposite, but they can still slip intravascularly if you are in a tight spot or unlucky

@@DrTimPearce IT ALL DEPENDS ON THE "SIZE" OF CANULA...A 22 - 25 g, "ROUND TIP", FLEXIBLE CANNULA IS MUCH, MUCH SAFER THAN A 27 OR A 30 g CANNULA!!! THE 27 - 30 g CANNULAS CAN VERY EASILY ACT MUCH LIKE A SHARP NEEDLE, WHEREAS, THE 22 - 25 g ONE WILL "SLIDE" RIGHT NEXT TO THE ARTERIES BUT NOT PENETRATE THEM...THE BIGGER, THE BETTER..."FLEXIBLE WITH SMOOTH, ROUND TIPS"!!!

I use hydrolonic pen to inject hydraulic acid into my lips. Great results. Seem both of you have similar personalities.

I've heard the opposite. That they inject with air pressure so the filler molecule is too big.
