I agree that doing bloodwork at the hospital is the most preferable. Hospital labs definitely are quicker than independent labs.

Next time you should check with the lab or google the order of the color of tubes you’re suppose to draw blood in just FYI.

Wouldn't they tell her if it needed to be in a certain order?
@lifeofazebra7738
@lifeofazebra7738 3 жыл бұрын
Miranda Stephens evidently they didn’t. Blood draws have to done in a certain order based on the color of the tube bc each tube has a different additive in it and drawing them out of order can effect the results. It should be light blue, red, gold, royal blue with serum, green, mint, royal blue without serum, lavender, gray.

As an RN I have to add a bit here - there are several different methods of drawing blood through a port and that often depends on the type of port you have but there are a few things that remain or should remain the same in every case. First. when flushing before a blood draw it needs to be at least 20 cc of saline which is usually 2 flush syringes, not one. This clears the line well of any thing else in the lines such as medications, heparin, or IV fluids. You cannot guess ta this and think "oh I have only been getting an IV with saline solution" or "there is only 5ccs of heparin in there for a hep lock" and so on. Anything less than 20 cc's of flush can end up messing up the results. You both should be wearing masks, due to the close confines of the car and how close you are to your own port connector you are breathing directly on it and contaminating it as soon as you get it clean. Holding your breath is not really a good technique plus you have an assistant also so close they should be masked. In most cases, depending on the length of the port tubing a mask non the patient, even if they are not working on the port, should be required. If the port is placed so that when the patient turns their head to teh side and holds it there while your working then they may not need a mask but the person working on the port, whether the patient or staff is going to be close enough and looking and therefore breathing directly on the port tubing and its connectors. When you realize that depending on the brand of Huber needle you use the tubing to the connector hub is only 6-10 inches and o that is how close, if not close that hub connector will be to your mouth. Even if I am drawing the blood on my daughter at home, safety says I wear a mask, and in hospital we are required to while drawing blood from a port, and that was long before Covid. When unsure when working on a port, put on a mask as its better to be safe and we always recommend that a patient who is maintaining their own port and therefore will have an exposed connector hub or clave connector within 10 inches of their face no matter what they are doing, should have a mask on. Especially if you are doing this in your vehicle and you have no running water in your car of course and because of your siting position and position of your arms and your tools and equipment you should fist of all, before anything else use alcohol based and sanitizer up to your elbows. At home you would be washing with soap and water correctly but that is not something you can do in a car. If hand sanitizer is not available then you should have sterile gloves with you - they won't remain sterile of course as the port tubing and the sample tubes and your other equipment is not sterile but by wearing them you are starting out with hands covered in a barrier that is much better then unwashed hands. This is not going to be a sterile or even a very clean procedure by a long shot, as you are in your vehicle but the more steps you take to eliminate any contamination by something being awkwardly touched accidentally and so on. For teh most part every attempt should be made to do the blood draw at home if you are having to do it yourself, and then transport the sample to the facility, you do not have to be on their doorstep to give blood. Most labs or you home health provider will provide you with a few tubes of each type, some transfer devices, 10 cc sterile syringes (empty), and you would have your own saline, heparin and alcohol and other supplies at home. Then when you doctor orders blood tests the lab can tell you what color tubes they will need and you can go from there. Just remember to check your tubes on a regular basis as they do expire and most labs will simply dispose of blood in and expired tube as not worth even running, so check teh date of your tubes and get fresh ones when you are within a month of their expiration date. As for attaching the vacuum tubes directly to the ports and having it pull blood out, that is really better off done by a professional who knows exactly what to look for In case ether is a small leak in the connections and you start getting air back. Also, you can easily damage a port connecting it to a vacuum tube and so most often its better for a patient to use a syringe to pull the blood back so that they can maintain less pull back pressure than the vacuum tube will as you have no way to control that except to pull the tube on and off the adapter and you really have to be experienced in doing that. In most cases we have the patient draw the blood out of the port with a 10 cc syringe or two and then attach it to the adapter (transfer device) and then insert the tube in device. When you see how fast the tube will pull the blood out of the syringe without you even depressing the plunger you can see what it might not, unless you are a professional and doing it all the time, the best of you to do it that way. As you are a home health nurse with a nursing background then you should be fine with directly using a vacuum tube but for the non medical person its not usually the safest method to teach or recommend and after reports of issues stemming from patients withdrawing blood in this manner themselves without the needed training it is not recommend that it be done that way and its much safer for the patient to control the pressure of the withdrawing of the blood by using a syringe. You are also trained well in aseptic techniques and should notice if you touch the wrong ting in the wrong place or accidentally contaminate something but that often is not the same for a home health care patient with no previous medical background. Still, contamination given the fact that a port is a direct communication with your blood system has a high chance of causing an infection and a vehicle, no matter how great your upkeep is is a notoriously dirty place. Simply the dust and particles that adhere to the interior roof as you use the car can be shaken loose and, while too small to be seem, be raining down on your workspace and syringe and port connections even once its cleaned with alcohol. Once you pull 5-7cc into the syringe as a discard amount there is not a need to also pull a discard amount into a vacuum tube to discard. When you are finished with drawing blood, flushed with 20ccs of saline you should lock the tubing, remove the clave or connection cap on the end of the port tubing using and aseptic/sterile technique, and then replace it with a new one primed with saline. if it was primed using a 10cc saline flush syringe and the syringe is still attached to keep the connector end sterile you can flush that saline through and either lock the tubing again or then flush with heparin and lock the tubing under pressure depending on the protocol you are on (not all clinics or home health care providers recommend a heplock anymore. If they are asking you to drawn your own blood the better move is to supply you with the needed tubes and ask you to draw it at home, put it on ice and transport it to the lab. Your home, where you normally access your port is going to be many levels cleaner than the front of the car in a parking lot and you have access to running water to wash well before accessing your tubing and manipulating your port as well as to all the equipment you would normally have on hand encase there was a problem with or accident with the port while doing this. Blood will remain in good condition for testing if iced immediately (unless its being drawn for something like blood culture at which it should not be) and then transported to the site it is to be tested. In an emergency if you have to this in the car I would suggest having some disposable sterile drapes about to put your supplies on in your lap and to use as a work area. Of course, its not going to stay sterile as you use it but at least it starts out that way and is not adding o any contamination your may be picking up. You can buy these in drapes easily from amazon in packages of 10 individually wrapped and some home health companies will also provide them. Also make sure that when labeling your tubes your accurate about the time of the draw and it is written prominently on the tubes. Blood samples can be held and are good for some time once in their respective tubes and many tubes even include preservatives but the age of the blood is often important of the lab to know as blood stored too long, whether on ice or not, does expire for some tests. When drawing at home you should try to take your sample in to the testing site within the hour, if possible so that the lab has as much time as possible with the blood in the event that a test must be completed withing a couple hours of a blood draw. Keep in mind that your sample may not be run immediately upon being given to the lab but wait in line behind others that arrived first, and also depending on on the type of test and the type of lab some samples may even have to be backed up and sent off to be tested, especially if using an independent lab or a off site satellite lab. Since most off site satellite labs and many of the independent labs all send the samples they get back to a main facility or the hospital lab they are a satellite of and that can delay both the time it takes for results and can sometimes cause issues with results themselves due to the amount of transport the sample goes through.

@@TheRawLife1 I hate it when people just want to judge based on what they want to hear, not on facts. We all only see a fraction of what goes on behind the scenes and we should never judge anyone, it's not okay. They don't like what they see, then they shouldn't watch or at least if they don't have anything nice to say, then they shouldn't say anything at all. But that's just me lol. Florida's coronavirus situation now is like NY was in April and May. I'm in NY so I understand what you're going through. After Cuomo initially screwed up with his nursing home plan, he seemed to embrace a better plan and now NY is actually doing quite well, and he has been very strict regarding mask wearing and social distancing. I hope Desantis gets on board (though I doubt it). Good luck!

I can agree when you don’t get blood return it can be so nerve racking and scary. I remember the first time that happened I had to stop and say breathe and try again. I do have a question though how did you find a lab that lets you draw your own. I was able to do it when I worked in the medical field and currently with COVID and my level of sickness I have a home draw nurse that comes out to the house. But I no longer can work and I need to find a lab where I can get them to give me the vials. I too live in Florida and most of my physicians are in Orlando and a few in Tampa. But I’d like to find a place to give me vials so I thought I’d ask you since I know you to live in central Florida.

It's so scary lol And as for the lab, we go to the lab directly inside a hospital and they are nice enough to give me the goal and adapter and bag. Quest will NOT do anything of this sort. They are way to strict. So maybe try a lab in a hospital or another lab besides quest. Go in and ask in person. Speak to a manager maybe ?
