Sanjay Sir I have created alert as you showed but it didn't trigger, I have restarted VM multiple times may I know the reason.

After creating Vm restart alert rule you should wait for 5-10 mins (after reboot). You will get Alert in azure monitor as well as on your mail or mobile number .
